Hello Dr.
Tsai,
I would like to ask you a question.
On a certain day at the end of 2017, after consuming citrus fruits, I experienced two symptoms that evening.
One symptom was a feeling of fullness and tightness in my lower right abdomen, occasionally accompanied by sharp pain and bowel sounds.
The other symptom was a persistent urge to pass gas but being unable to do so, with a sensation of pressure and fullness at the anal area.
Nearly six years have passed since then, during which I have sought treatment from both traditional Chinese and Western medicine.
The issue of the persistent urge to pass gas has since improved and occurs less frequently; however, the feeling of fullness and tightness in my lower right abdomen, along with occasional sharp pain and bowel sounds, still occurs quite often.
I have the following questions for Dr.
Tsai:
Q1.
Based on the symptoms I experienced after consuming citrus fruits at the end of 2017, what gastrointestinal issue do you think I might have? Additionally, why has it not completely resolved after nearly six years?
Q2.
Following up on the previous question, could my symptoms possibly be indicative of "Irritable Bowel Syndrome" (IBS)? Given that my symptoms of fullness, sharp pain, and bowel sounds in the lower right abdomen still occur intermittently, while my bowel movements remain normal (once a day).
Thank you, Dr.
Tsai.

Hello: The symptoms you are experiencing may simply be a result of food irritating the gastrointestinal tract, leading to increased gastrointestinal motility.

Chronic gastrointestinal symptoms can be quite challenging to diagnose and manage, especially when they persist over several years. Based on your description, it seems that you have been experiencing discomfort in your right lower abdomen, characterized by a feeling of tightness, occasional sharp pains, and bowel sounds, along with a sensation of incomplete gas release. These symptoms have been ongoing since late 2017, following the consumption of citrus fruits.

Q1: Possible Gastrointestinal Issues
The symptoms you describe could be indicative of several gastrointestinal issues. One possibility is Irritable Bowel Syndrome (IBS), which is a functional gastrointestinal disorder characterized by abdominal pain and altered bowel habits without any identifiable organic cause. IBS can manifest with symptoms such as bloating, gas, and abdominal discomfort, which can be exacerbated by certain foods, stress, or hormonal changes. The fact that your symptoms began after eating citrus may suggest a food sensitivity or intolerance, which is common in individuals with IBS.

Another potential issue could be chronic constipation or functional dyspepsia, where the sensation of incomplete evacuation or gas retention can lead to discomfort and abdominal pain. Additionally, the presence of bowel sounds may indicate increased intestinal motility, which can occur in response to dietary triggers or stress.

The persistence of your symptoms over six years could be attributed to several factors, including dietary habits, stress levels, and possibly an underlying functional disorder that has not been adequately addressed. Chronic gastrointestinal symptoms often require a multifaceted approach to treatment, including dietary modifications, stress management, and sometimes medication.

Q2: Is It Irritable Bowel Syndrome?
Given the chronic nature of your symptoms and the fact that your bowel movements are regular (once daily), it is plausible that you may be experiencing IBS. IBS is often diagnosed based on symptom patterns rather than specific tests, and it can be classified into different subtypes based on the predominant symptoms (IBS with constipation, IBS with diarrhea, or IBS mixed). Your description of abdominal discomfort and the sensation of gas retention aligns with the characteristics of IBS.

To further evaluate your condition, it may be beneficial to keep a symptom diary that tracks your food intake, stress levels, and symptom occurrences. This can help identify potential triggers and patterns that may be contributing to your discomfort. Additionally, consulting with a gastroenterologist for further evaluation, including possibly undergoing tests such as a colonoscopy or imaging studies, may help rule out other conditions such as inflammatory bowel disease (IBD) or structural abnormalities.

Management Strategies
1. Dietary Modifications: Consider adopting a low-FODMAP diet, which has been shown to help many individuals with IBS. This involves reducing certain carbohydrates that can ferment in the gut and cause symptoms.

2. Stress Management: Since stress can exacerbate gastrointestinal symptoms, incorporating relaxation techniques such as mindfulness, yoga, or cognitive behavioral therapy may be beneficial.

3. Medications: Depending on the severity of your symptoms, medications such as antispasmodics, laxatives, or medications specifically for IBS may be prescribed by your healthcare provider.

4. Regular Follow-ups: Continuous monitoring and follow-up with your healthcare provider can help adjust your treatment plan as needed and ensure that any new symptoms are addressed promptly.

In conclusion, while your symptoms may suggest IBS or another functional gastrointestinal disorder, a thorough evaluation by a healthcare professional is essential for accurate diagnosis and effective management. It is important to remain proactive in seeking care and advocating for your health, as chronic gastrointestinal issues can significantly impact your quality of life.
