Ejaculatory dysfunction?
Hello doctor, I am currently only 20 years old, but I have noticed that during ejaculation, the semen flows out rather than being expelled, and the volume seems to be less compared to others.
I ejaculate approximately every two to three days, and it always flows out.
If I hold off for a week, I can ejaculate forcefully, but only once; after the first ejaculation, the subsequent releases are still flowing or dripping.
I would like to know if this could be indicative of any underlying medical conditions that would require a clinic visit.
I have read that some patients with spinal injuries experience sexual dysfunction, and I have scoliosis.
Could this also be affecting my ejaculation issues?

Dr. Cai Zhuorong reply Urology
If ejaculation is not accompanied by discomfort, treatment is generally not necessary.
Scoliosis, in the absence of nerve compression, typically does not have an impact.

Ejaculation issues, particularly in young men, can be a source of concern and confusion. It's important to understand that various factors can contribute to these problems, including physiological, psychological, and lifestyle elements. Based on your description, it seems you are experiencing a form of ejaculation that is more fluid than forceful, and you have concerns about the volume of semen produced. Let's break down the potential causes and solutions for your situation.
Potential Causes
1. Physiological Factors:
- Anatomical Issues: Conditions such as spinal cord injuries or anatomical abnormalities can affect ejaculation. You mentioned having scoliosis, which in some cases can impact nerve function and may contribute to sexual dysfunction. However, this is not always the case, and many individuals with scoliosis do not experience such issues.
- Hormonal Levels: Testosterone plays a critical role in sexual function, including libido and ejaculation. Low testosterone levels can lead to reduced semen volume and changes in ejaculation. A blood test can help determine if your hormone levels are within the normal range.
2. Psychological Factors:
- Anxiety and Stress: Psychological factors, including anxiety about sexual performance or general stress, can significantly impact sexual function. If you are worried about your performance or have experienced anxiety related to sexual activity, this could lead to difficulties in ejaculation.
- Conditioning: If you have developed a pattern of ejaculation that is more fluid due to frequent masturbation or specific sexual practices, this could condition your body to respond in that way.
3. Lifestyle Factors:
- Frequency of Ejaculation: You mentioned that you ejaculate every two to three days. While this frequency is generally considered normal, individual responses can vary. If you find that waiting longer leads to a more forceful ejaculation, it might be beneficial to adjust your frequency to see if it makes a difference.
- Diet and Hydration: A poor diet or dehydration can affect semen volume. Ensuring you are well-hydrated and consuming a balanced diet rich in vitamins and minerals can support overall sexual health.
Solutions and Recommendations
1. Consult a Healthcare Professional: Given your concerns, it would be wise to consult a urologist or a healthcare provider specializing in sexual health. They can perform a thorough evaluation, including physical examinations and possibly blood tests to check hormone levels.
2. Address Psychological Factors: If anxiety or stress is a concern, consider speaking with a mental health professional. Techniques such as cognitive-behavioral therapy (CBT) can help address performance anxiety and improve your sexual confidence.
3. Lifestyle Adjustments:
- Diet and Exercise: Focus on a healthy diet, regular exercise, and adequate hydration. Foods rich in zinc (like nuts and seeds) and antioxidants (like fruits and vegetables) can support reproductive health.
- Masturbation Practices: Experiment with the frequency of masturbation. If you find that waiting longer leads to a more satisfying experience, consider adjusting your routine accordingly.
4. Education and Awareness: Understanding normal sexual function can alleviate some of the anxiety surrounding performance. Remember that variations in ejaculation are common, and many factors can influence this.
5. Follow-Up: If you notice persistent issues or if your concerns escalate, do not hesitate to seek further medical advice. Regular check-ups can help monitor your sexual health and address any emerging concerns.
In conclusion, while your situation may feel concerning, it is essential to approach it with a comprehensive understanding of the potential causes and solutions. By consulting with healthcare professionals and making informed lifestyle choices, you can work towards improving your sexual health and overall well-being.
