Ejaculatory dysfunction?
Hello doctor, I am currently only 20 years old, but I have noticed that during ejaculation, the semen flows out rather than being expelled, and the amount seems to be less compared to others.
I ejaculate about every two to three days, and it always flows out.
If I hold off for a week, I can ejaculate forcefully, but only once; after the first ejaculation, the rest is still just flowing or dripping.
I would like to know if this could indicate any underlying medical conditions that would require a clinic visit.
I have read that some patients with spinal injuries experience sexual dysfunction, and I have scoliosis.
Could this also be affecting my ejaculation issues?

If ejaculation is not accompanied by discomfort, treatment is generally not necessary.
Scoliosis, if it does not compress nerves, will generally not have an impact.

Ejaculation issues in young men can be a source of concern and confusion, especially when they experience changes in their sexual function. At the age of 20, it's important to understand that various factors can contribute to these issues, including physiological, psychological, and lifestyle influences.
Firstly, the phenomenon of "flowing" versus "shooting" during ejaculation can be attributed to several factors. The force and volume of ejaculation are influenced by the contraction of the pelvic floor muscles and the prostate gland's ability to expel semen. If you notice that your ejaculation is more of a flow rather than a forceful shoot, it could be due to a few reasons:
1. Pelvic Floor Muscle Tone: The strength of the pelvic floor muscles plays a crucial role in ejaculation. Weakness in these muscles can lead to less forceful ejaculation. Engaging in pelvic floor exercises, often referred to as Kegel exercises, can help strengthen these muscles and potentially improve the force of ejaculation.
2. Prostate Health: The prostate gland is responsible for producing a significant portion of the fluid in semen. If there are any issues with the prostate, such as inflammation or infection (prostatitis), it could affect the volume and force of ejaculation. Regular check-ups with a healthcare provider can help monitor prostate health.
3. Hormonal Factors: Testosterone levels can impact sexual function, including ejaculation. If there are concerns about hormonal imbalances, a healthcare provider can perform tests to evaluate hormone levels and determine if there is a need for intervention.
4. Psychological Factors: Anxiety, stress, and psychological pressure can significantly affect sexual performance. If you are feeling anxious about your sexual function, this could lead to a cycle of performance anxiety, which may further exacerbate the issue. It may be beneficial to explore relaxation techniques, counseling, or therapy to address these psychological factors.
5. Physical Conditions: You mentioned having scoliosis, which can potentially impact nerve function and pelvic alignment. While scoliosis itself may not directly cause ejaculation issues, any spinal condition that affects nerve pathways could theoretically influence sexual function. Consulting with a healthcare provider who specializes in sexual health or a urologist may provide insights into whether your spinal condition is contributing to your symptoms.
Regarding the volume of semen, it's important to note that individual variations exist. Factors such as hydration, frequency of ejaculation, and overall health can influence semen volume. If you are ejaculating less frequently (every two to three days), this could also affect the volume and force of ejaculation.
If you are concerned about your symptoms, it is advisable to seek medical advice. A healthcare professional can conduct a thorough evaluation, which may include a physical examination, hormone level testing, and possibly imaging studies if deemed necessary. They can help rule out any underlying medical conditions and provide tailored recommendations based on your specific situation.
In summary, ejaculation issues can stem from a combination of physical, psychological, and lifestyle factors. Engaging in pelvic floor exercises, managing stress, maintaining a healthy lifestyle, and seeking professional medical advice can all contribute to improving your sexual health and function. Remember, you are not alone in facing these challenges, and there are effective solutions available.
