Arrhythmia issues?
Hello doctor, last night I felt a bit of palpitations while I was sleeping.
I placed my hand on my carotid artery to check my pulse and noticed that it was normal at first, but then I suddenly felt a triplet of beats, and afterward my heart rate increased to 140.
Aside from the rapid heartbeat, I did not experience any other discomfort, so I went to the emergency room.
The ECG in the emergency department showed no issues.
I would like to ask, what type of arrhythmia could cause such sudden triplet beats, and is there a risk of sudden cardiac death? Thank you, doctor.

To determine if there is an arrhythmia, it is necessary to go to the hospital for an electrocardiogram (ECG) examination.

Palpitations, or the sensation of feeling your heart beating rapidly or irregularly, can be concerning, especially when they occur suddenly and are accompanied by a significant increase in heart rate, as you described. Your experience of feeling a "three-beat" sensation followed by a rapid heart rate of 140 beats per minute is indeed noteworthy and warrants further understanding.
The sensation you experienced could be indicative of a type of arrhythmia known as "premature ventricular contractions" (PVCs) or "premature atrial contractions" (PACs). These are common types of arrhythmias that can cause a feeling of a skipped beat or a "fluttering" sensation in the chest. In the case of PVCs, the heart may beat irregularly due to an extra electrical impulse originating from the ventricles, while PACs arise from the atria. Both conditions can lead to the sensation of a "three-beat" pattern, especially if they occur in quick succession.
It's reassuring that your emergency department visit resulted in a normal electrocardiogram (ECG), which suggests that there were no immediate life-threatening arrhythmias detected at that time. However, it is essential to understand that some arrhythmias can be intermittent, meaning they may not always be captured on an ECG during a brief examination. If you continue to experience palpitations, it may be beneficial to undergo further monitoring, such as a Holter monitor, which records your heart's activity over 24 hours or longer. This can help identify any irregularities that may occur outside of a clinical setting.
Regarding your concern about the risk of sudden cardiac death, it is important to note that while some arrhythmias can be serious, the majority of palpitations are benign, especially in individuals without underlying heart disease. Factors such as anxiety, caffeine intake, dehydration, and stress can also contribute to palpitations. If you have no history of heart disease and your tests are normal, the likelihood of a serious condition is significantly reduced.
However, it is crucial to remain vigilant. If you experience additional symptoms such as chest pain, shortness of breath, dizziness, or fainting, you should seek immediate medical attention. These symptoms could indicate a more serious underlying condition that requires further evaluation.
In summary, while your experience of palpitations can be alarming, it is essential to consider the context and any accompanying symptoms. Regular follow-ups with your healthcare provider, lifestyle modifications (such as reducing caffeine and managing stress), and possibly further monitoring can help manage your symptoms and provide peace of mind. If you have ongoing concerns, do not hesitate to discuss them with your doctor, who can provide personalized advice based on your health history and current condition.
