After hemorrhoid surgery, should the ointment be applied internally?
Hello, Doctor.
I underwent a Grade IV mixed hemorrhoid surgery on December 15, where both internal and external hemorrhoids were completely excised.
I opted for the Ligasure minimally invasive technique.
After the surgery, my attending physician prescribed a topical ointment called "Le Yu Zhi" and an antibiotic ointment.
The doctor instructed me to apply the Le Yu Zhi ointment half a finger's depth into the anal canal.
However, after several attempts, I found it difficult to reach that depth, and I noticed blood on my finger afterward.
I'm also unsure if the ointment is actually reaching the internal wound.
I would like to ask if it is really necessary to apply medication deep inside the anal canal for post-operative care of internal hemorrhoids?

Dr. Zhang Shuhao reply Surgery
Postoperative ointments have antibacterial and soothing effects on wounds.
However, if the wound is prone to bleeding due to its depth, you can apply the ointment only to the outer edges to minimize irritation to the wound.
Bleeding will gradually improve as the wound heals.

After undergoing a fourth-degree mixed hemorrhoidectomy, it is understandable to have concerns about post-operative care, especially regarding the application of ointments internally. The use of hemorrhoid ointments, such as the one prescribed by your physician, is often recommended to promote healing and alleviate discomfort. However, the method of application can be a source of confusion and discomfort, particularly after surgery.
In general, the application of ointments internally after hemorrhoid surgery is intended to help soothe the surgical site, reduce inflammation, and prevent infection. The specific instructions to apply the ointment "half a finger's length" inside the rectum are aimed at ensuring that the medication reaches the affected area effectively. However, it is crucial to approach this with caution, especially if you are experiencing bleeding upon application.
Here are some important considerations regarding the application of hemorrhoid ointment after surgery:
1. Gentle Application: It is essential to apply the ointment gently. If you are experiencing bleeding when attempting to insert your finger, it may indicate that you are applying too much pressure or that the area is still very sensitive post-surgery. You might want to try a smaller amount of ointment or use a cotton swab for application instead of your finger to minimize discomfort.
2. Consult Your Physician: If you find that you are unable to apply the ointment as instructed without causing bleeding, it is crucial to consult your surgeon or healthcare provider. They can assess whether the bleeding is a normal part of the healing process or if there is a concern that needs to be addressed. They may also provide alternative methods for applying the ointment or suggest a different treatment plan.
3. Healing Process: Keep in mind that after surgery, the tissues in the anal area are healing, and it is not uncommon to experience some discomfort or sensitivity. The presence of blood on your finger after application could be a sign of irritation rather than a complication, but it should not be ignored.
4. Follow-Up Care: Regular follow-up appointments with your healthcare provider are essential to monitor your healing progress. During these visits, you can discuss any difficulties you are experiencing with medication application and receive tailored advice based on your recovery.
5. Alternative Treatments: If applying the ointment internally continues to be problematic, your physician may recommend alternative treatments or adjustments to your post-operative care regimen. This could include oral medications, sitz baths, or other topical treatments that may be easier to apply.
6. Signs of Complications: Be vigilant for any signs of complications, such as increased pain, persistent bleeding, or signs of infection (such as fever or unusual discharge). If you experience any of these symptoms, seek medical attention promptly.
In conclusion, while applying hemorrhoid ointment internally after surgery is often recommended for optimal healing, it is essential to do so gently and with care. If you encounter difficulties or discomfort, do not hesitate to reach out to your healthcare provider for guidance. They can provide you with the best advice tailored to your specific situation and ensure that your recovery progresses smoothly.
