Acne developed on a scar?

Hello, doctor.
I had a car accident over a month ago, resulting in a nearly 20 cm sutured wound on the back of my calf.
It has healed, and I am currently using silicone scar sheets.
Today, when I removed the sheet, I noticed a pimple filled with pus at the scar site (I did not notice it the day before).
After I squeezed it, the contents were a mixture of blood and pus, and the area is now indented.
I have cleaned it and applied a hydrocolloid dressing.
Is this situation normal, or do I need to return for a medical evaluation?

Dr. Lu Daokuan reply Plastic Surgery

It may be a localized infection at the suture site; a follow-up appointment is recommended.

Dealing with pimples on scars, especially after surgery, can be concerning and requires careful attention. In your case, having developed a pimple filled with pus on a healing scar from a surgical wound is not uncommon, but it does warrant some caution and proper management.

Firstly, it’s important to understand that the skin undergoes various changes during the healing process. After surgery, the skin is particularly vulnerable to infections and other complications, such as the formation of pimples or cysts. The presence of a pimple filled with pus indicates that there may be an infection or inflammation occurring in the area. This can happen due to several reasons, including irritation from the silicone gel sheets, friction, or even bacteria entering through the skin.

Here are some steps you can take to manage the situation:
1. Keep the Area Clean: Ensure that the area around the scar is kept clean. Use a gentle cleanser to wash the area, and avoid using harsh scrubs or products that could irritate the skin further.

2. Avoid Picking: It’s crucial to avoid picking at the pimple or the scar. While it may be tempting to squeeze out the pus, this can lead to further irritation, potential scarring, or even a more serious infection.

3. Monitor for Signs of Infection: Keep an eye on the area for any signs of worsening infection, such as increased redness, swelling, warmth, or discharge. If you notice these symptoms, it may be necessary to seek medical attention.

4. Use Appropriate Dressings: Since you mentioned using artificial skin, ensure that it is applied correctly and is not causing additional irritation. Artificial skin can help protect the area and promote healing, but it should be changed regularly to prevent moisture buildup, which can lead to infection.

5. Consider Topical Treatments: Depending on the severity of the pimple, you might consider using over-the-counter topical treatments that contain benzoyl peroxide or salicylic acid. However, consult with a healthcare professional before applying any new products to the area, especially since it is healing from surgery.

6. Consult a Healthcare Professional: Given that you have already experienced a significant surgical wound and now have developed a pimple with pus, it would be wise to consult with your healthcare provider. They can assess the area and determine if there is an infection that requires treatment, such as antibiotics.

7. Follow-Up Care: If you have not already scheduled a follow-up appointment, it would be beneficial to do so. Regular follow-ups can help monitor the healing process and address any complications early on.

In summary, while the appearance of a pimple on a healing scar can be a normal occurrence, it is essential to manage it carefully to prevent complications. Keeping the area clean, avoiding irritation, and monitoring for signs of infection are key steps. If you have any concerns or if the situation worsens, do not hesitate to seek medical advice. Your health and recovery should always be the priority.
