Red bumps under the foreskin?

Hello Dr.
Du, I have noticed two red spots on either side of my penis beneath the retractable foreskin.
The red, swollen areas are about the size of blisters and are painful when pressed.
Additionally, there is a persistent odor coming from my penis.
I'm unsure if this is related to these symptoms.
Do you have any recommended medications and advice on how to manage this condition?

xx, 20~29 year old female. Ask Date: 2023/12/29

Dr. Du Shixi reply Urology

Hello: It could possibly be eczema.
If there has been any unsafe sexual behavior, please seek medical attention.
Additionally, it is important to keep the area dry, as the foreskin can retain moisture.
If there is any moisture present, make sure to dry it thoroughly.
If there is no improvement after three days, please consult a doctor.
We cannot recommend any medications in this setting.
Wishing you good health.

When it comes to red bumps under the foreskin, there are several potential causes that could be contributing to your symptoms. The presence of red, swollen bumps that resemble blisters, along with pain upon pressure and an unusual odor, can indicate a few different conditions, including infections, inflammatory skin conditions, or sexually transmitted infections (STIs).
One common cause of such symptoms is balanitis, which is an inflammation of the glans (the head of the penis) and can occur due to poor hygiene, irritation from soaps or lotions, or infections (both fungal and bacterial). Another possibility is a sexually transmitted infection, such as herpes or genital warts, which can present as red bumps or blisters and may be accompanied by pain and discharge.

Given the symptoms you described, it is crucial to seek medical attention from a healthcare provider, preferably a dermatologist or urologist, who can perform a physical examination and possibly run tests to determine the exact cause of your symptoms. Self-diagnosing or self-treating can lead to complications or worsening of the condition.

In terms of treatment options, the approach will depend on the underlying cause. If it is determined to be an infection, your doctor may prescribe topical or oral antifungal or antibiotic medications. For inflammatory conditions, corticosteroid creams may be recommended to reduce inflammation and discomfort. If an STI is diagnosed, appropriate antiviral or antibiotic treatment will be necessary.

In the meantime, here are some general recommendations to help manage your symptoms:
1. Maintain Hygiene: Gently clean the area with mild soap and water daily. Avoid using harsh soaps or products that can irritate the skin.

2. Avoid Irritants: Stay away from scented soaps, lotions, or other products that may cause irritation.
3. Wear Loose Clothing: Opt for breathable, loose-fitting underwear and clothing to reduce friction and moisture buildup.

4. Avoid Sexual Activity: Until you have been evaluated by a healthcare provider, it is advisable to refrain from sexual activity to prevent potential transmission of infections and to avoid further irritation.

5. Over-the-Counter Treatments: While waiting for your appointment, you might consider using over-the-counter hydrocortisone cream to reduce inflammation, but this should only be a temporary measure and not a substitute for professional medical advice.

6. Monitor Symptoms: Keep track of any changes in your symptoms, including the appearance of new bumps, changes in odor, or any discharge, as this information will be helpful for your healthcare provider.

It is essential to address these symptoms promptly, as untreated infections or inflammatory conditions can lead to more severe complications. Always consult with a healthcare professional for an accurate diagnosis and appropriate treatment plan tailored to your specific situation.
