Chest tightness, insomnia?

Hello, doctor.
I would like to ask what diseases or heart conditions could cause left chest tightness and insomnia.
1.
I am currently having difficulty falling asleep and rely on Eszopiclone and antiarrhythmic medication to help me sleep.
After falling asleep, I consistently wake up around 2-3 AM to urinate, and when I lie back down, I feel a tightness in my left chest, which is uncomfortable.
Sometimes, my hands and feet feel numb, and I cannot fall back asleep until morning.
This also happens when I sit with my eyes closed for a long time, especially when I am about to fall asleep or after lying in bed for a while.
During the day, I occasionally feel tightness in my left chest.
(I have hypertension and was originally taking Candesartan, but my doctor switched me to antiarrhythmic medication, yet my symptoms have not improved.) 2.
I have undergone an electrocardiogram, a 24-hour Holter monitor, an echocardiogram, and a stress test (which showed positive results at stages 3 and 4), and a cardiac catheterization, which indicated that one artery is 10% blocked.
3.
In October, I was able to fall asleep, but as soon as I fell asleep, my heart would startle me awake.
This led to insomnia, and after undergoing tests, I had a cardiac catheterization in December.
Now, I wake up every morning to urinate, and when I lie back down, I feel discomfort and tightness in my left chest (sometimes accompanied by a sensation of blood swirling and numbness in my hands and feet).

Dr. He Dongjin reply Cardiology

Hello, it doesn't seem to be a heart issue.
A cardiac catheterization showed that one blood vessel is 10% blocked, which is not concerning.
It may be related to cervical spine issues or autonomic nervous system problems.
I recommend seeing a neurologist for further evaluation.
Take care from Changhua Hospital.

Understanding the symptoms of chest discomfort and insomnia, particularly in the context of heart health, is crucial for both diagnosis and management. Your symptoms of left chest discomfort, insomnia, and associated sensations like numbness in the hands and feet can be indicative of several underlying issues, particularly given your history of hypertension and previous cardiac evaluations.

1. Chest Discomfort and Insomnia: The left chest discomfort you are experiencing could be related to a variety of factors. Given your history of hypertension and previous cardiac evaluations, it is essential to consider both cardiac and non-cardiac causes. Cardiac issues such as angina, which is chest pain due to reduced blood flow to the heart, can manifest as discomfort in the chest, particularly on exertion or stress. However, since your recent cardiac evaluations, including echocardiograms and stress tests, have shown only mild issues (like a 10% blockage), it is less likely that your symptoms are solely due to significant coronary artery disease.

Non-cardiac causes could include gastroesophageal reflux disease (GERD), which can cause chest discomfort that mimics heart pain, or anxiety and panic disorders, which are known to cause both chest discomfort and insomnia. Anxiety can lead to hyperventilation, which may cause tingling or numbness in the extremities, further complicating your symptoms.

2. Insomnia and Sleep Disturbances: The insomnia you are experiencing, particularly the pattern of waking up in the early morning hours, can be related to several factors. Stress and anxiety can significantly impact sleep quality, leading to difficulties in both falling asleep and staying asleep. The sensation of your heart "waking you up" could be related to anxiety or panic attacks, which can occur during sleep and cause abrupt awakenings.
Additionally, the use of medications such as Eszopiclone (Lunesta) and beta-blockers (like the heart rhythm medication you mentioned) can also influence sleep patterns. While beta-blockers are often prescribed for hypertension and heart rhythm issues, they can sometimes cause sleep disturbances or vivid dreams, which might contribute to your insomnia.

3. Management Strategies: Given your symptoms and medical history, it is essential to adopt a multifaceted approach to management:
- Cardiac Follow-Up: Continue to work closely with your cardiologist to monitor your heart health. If your symptoms persist or worsen, further evaluation may be warranted, including potential imaging or stress testing.

- Mental Health Support: Consider consulting with a mental health professional. Cognitive-behavioral therapy (CBT) has been shown to be effective for insomnia and anxiety. Learning relaxation techniques, such as mindfulness or deep breathing exercises, can also help manage anxiety and improve sleep quality.

- Lifestyle Modifications: Incorporate lifestyle changes that promote heart health and better sleep. Regular physical activity, a balanced diet, and good sleep hygiene practices (such as maintaining a regular sleep schedule and creating a restful sleep environment) can be beneficial.

- Medication Review: Discuss with your physician the possibility of adjusting your medications. If you are experiencing side effects from your current medications, there may be alternatives that can help manage your symptoms without impacting your sleep.

In conclusion, while your symptoms of left chest discomfort and insomnia may have a cardiac component, they could also be significantly influenced by anxiety and lifestyle factors. A comprehensive approach that includes medical evaluation, mental health support, and lifestyle changes is essential for improving your overall well-being. Always consult with your healthcare provider before making any changes to your treatment plan.
