Postpartum issues, postpartum hemorrhage?

Hello, Director Lin.
I am an elderly postpartum woman.
I gave birth at the end of June last year.
After delivery, my lochia was not excessive, just a normal menstrual flow, and there were not many blood clots.
The lochia lasted for about half a month.
My obstetrician said this was normal.
At my one-month postpartum check-up, she mentioned that my uterus was recovering quickly and that the lochia was clearing out well, but I had fibroids, which were not affecting me.
She advised me to return for a follow-up in a year.
However, in August and September postpartum, my menstrual cycle became irregular.
Sometimes my period would last three to four days and then stop, with a lighter flow than before.
Prior to giving birth, my cycle was 28 days with menstruation lasting seven days.
By November, my cycle was about 30 days with seven days of bleeding, and the flow was similar to what it was before pregnancy.
But in December, the flow increased significantly, requiring me to use overnight pads, and the blood clots became larger and more frequent.
Whenever I stood up, it felt like water was flowing out, and the toilet would be filled with blood and clots.
This month has been the same.
Due to taking care of my child, I have no one to help me, so I haven't been able to go to the hospital for an examination.
I tried searching online for information but couldn't find anything.
I happened to come across this website and thought I would inquire about my situation.
I would like to ask Director Lin if this is normal or if there is something wrong with me.
Please help me identify the issue.
Thank you, Director.

Hello: Please visit for an examination, as I cannot provide an online response.
The situation may be due to fibroids or could be caused by postpartum hormonal imbalance.

Postpartum concerns, especially regarding heavy bleeding and irregular menstrual cycles, can be quite distressing for many women. Based on your description, it seems you are experiencing a significant change in your menstrual patterns following childbirth, which is not uncommon but can be concerning.

Firstly, it's important to understand that postpartum bleeding, known as lochia, typically occurs after delivery as the body expels the remnants of the placenta and uterine lining. This bleeding can last for several weeks and varies in volume and duration among women. In your case, it sounds like your lochia was within normal limits initially, but the subsequent changes in your menstrual cycle are raising concerns.

1. Heavy Menstrual Bleeding: The increase in menstrual bleeding you are experiencing, particularly the need for larger sanitary products and the presence of blood clots, could indicate a few potential issues. One possibility is that you may have retained placental tissue, which can lead to prolonged bleeding and heavier menstrual cycles. This condition can occur if the placenta does not fully detach during delivery or if remnants remain in the uterus, leading to complications such as infection or abnormal uterine bleeding.

2. Irregular Cycles: After childbirth, it is common for women to experience irregular menstrual cycles as the body readjusts to hormonal changes. Factors such as breastfeeding can also influence menstrual regularity. However, the significant increase in blood volume and the presence of large clots suggest that there may be an underlying issue that needs to be addressed.

3. Potential Causes: Other potential causes of heavy menstrual bleeding and irregular cycles include:
- Uterine Fibroids: You mentioned that your doctor noted the presence of fibroids, which can contribute to heavy bleeding and changes in menstrual patterns. Fibroids can cause increased menstrual flow and may require monitoring or treatment if they become problematic.

- Hormonal Imbalances: Fluctuations in hormones postpartum can lead to irregular cycles. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can also affect menstrual regularity and flow.

- Coagulation Disorders: If you have a history of bleeding disorders or if heavy bleeding is a new symptom, it may be worthwhile to investigate potential clotting issues.

4. When to Seek Help: Given the severity of your symptoms—especially the heavy bleeding and the presence of clots—it is crucial to seek medical attention. While online resources can provide general information, they cannot replace a thorough evaluation by a healthcare professional. You should consider scheduling an appointment with your gynecologist or a healthcare provider as soon as possible. They may recommend tests such as a pelvic ultrasound to check for retained tissue or fibroids, blood tests to assess your hemoglobin levels, and possibly a hysteroscopy to directly visualize the uterine cavity.

5. Managing Symptoms: In the meantime, managing your symptoms is essential. Ensure you stay hydrated, and consider using over-the-counter pain relief if you experience discomfort. If you notice any signs of severe bleeding (soaking through a pad in an hour), dizziness, or fainting, seek emergency medical care immediately.

In conclusion, while some irregularity in menstrual cycles postpartum is normal, the extent of your symptoms suggests that further evaluation is warranted. Don't hesitate to reach out to your healthcare provider to discuss your concerns and explore potential treatment options. Your health and well-being are paramount, especially as you navigate the challenges of motherhood.
