Coughing issues?

Hello Doctor: I have been coughing continuously since early December (for about a month now).
I have visited various clinics and taken medications, but there has been little improvement.
I also consulted a pulmonologist and had an X-ray, which showed no issues.
Although I have allergies, I have never experienced similar problems in previous years.
I would like to ask the doctor about an incident on the day my cough started; I had a haircut, and a hair strand got stuck in my throat.
I coughed forcefully at that time, but I'm not sure if I expelled the hair or swallowed it.
Since that day, I have been coughing.
Would it be advisable to have a bronchoscopy to check if the hair has entered my airway? Additionally, this morning, when I tried to cough, I accidentally pressed on the lower right side of my throat and felt an itching sensation and the urge to cough.
After pressing, I no longer felt the need to cough.
I would like to know what might be causing this issue.
Thank you for your assistance, Doctor!

Hello: Most people will feel an itchy cough when pressing below the throat.
I have not encountered any cases of hair entering the trachea; if that were the case, there would likely be persistent and unbearable coughing.
Your cough is probably intermittent and may be related to significant temperature changes in the weather or consuming cold raw fruits and vegetables.
I recommend drinking warm water from a thermos!

Persistent coughing can be a frustrating and concerning symptom, especially when it lasts for an extended period, as in your case. Given the details you've provided, let's break down the potential causes and considerations for your situation.

Firstly, it's important to note that a persistent cough lasting more than a month warrants a thorough evaluation. Since you've already seen various clinics and a pulmonologist without significant findings on X-rays, we need to consider other possibilities. The fact that you mentioned a hair potentially entering your throat during a haircut is intriguing. While it is unlikely that a hair would cause a persistent cough for over a month, it is not impossible for foreign bodies to cause irritation or inflammation in the airways, leading to a cough.

The sensation of itching and the urge to cough when pressing on the area of your throat could indicate localized irritation or inflammation. This could be due to several factors, including allergies, post-nasal drip, or even gastroesophageal reflux disease (GERD), which can cause a chronic cough. Allergies can lead to increased mucus production, which may drip down the back of your throat and trigger a cough reflex. If you have a history of allergies, this could be a significant factor in your persistent cough.

Considering your symptoms and the duration of your cough, a bronchoscopy could be a reasonable next step. This procedure allows a pulmonologist to directly visualize the airways and assess for any foreign bodies, inflammation, or other abnormalities that may not be evident on imaging studies. It is a minimally invasive procedure and can provide valuable information regarding the underlying cause of your cough.

In addition to a bronchoscopy, it may be beneficial to explore other diagnostic tests. These could include allergy testing, pulmonary function tests, or even a trial of medications for GERD if that is suspected. If allergies are a concern, identifying specific triggers can help manage your symptoms more effectively.

In the meantime, consider some supportive measures to alleviate your cough. Staying hydrated, using a humidifier, and avoiding known allergens or irritants (like smoke or strong odors) can help soothe your throat and reduce coughing. Over-the-counter antihistamines may also provide relief if allergies are contributing to your symptoms.

Lastly, if you experience any new symptoms, such as difficulty breathing, chest pain, or coughing up blood, seek immediate medical attention. These could be signs of a more serious condition that requires prompt evaluation.

In summary, while the hair incident may not be the direct cause of your persistent cough, it is essential to investigate further to rule out any underlying issues. A bronchoscopy could be a valuable tool in this process, and addressing potential allergies or reflux may also provide relief. Don't hesitate to follow up with your pulmonologist to discuss these options and ensure you receive the appropriate care.
