Post-Hemorrhoid Surgery Care: When to Stop Using Ointment? - Surgery

Post-hemorrhoidectomy

Hello, doctor.
It has been 43 days since my hemorrhoid surgery.
I only had a follow-up appointment 7 days post-surgery, and currently, there are no significant issues with the wound.
However, I have a question: since there has been no discharge from the wound and the stitches just fell out a few days ago (37 days post-surgery), I have been applying the neomycin ointment prescribed by the doctor but am unsure when to stop.
I would like to know if I can discontinue the ointment now that there is no discharge, and I am not experiencing any bleeding or pain after bowel movements.
Additionally, I sometimes feel a slight itching and discomfort due to the wound tightening.
Is this related to the ointment application or because the wound has not fully healed yet? Can I purchase over-the-counter hemorrhoid ointment for use? Thank you, doctor, for taking the time to read and respond.

Dr. Zhang Shuhao reply Surgery

If the wound has healed and is not painful, there is no need to use ointment.
When the wound feels slightly itchy or uncomfortable due to contraction, a warm water sitz bath can be helpful.
Hemorrhoid ointments can alleviate symptoms, but after surgery, it is advisable to choose products that do not contain steroids to avoid affecting the healing process.

After hemorrhoid surgery, proper care is crucial for a smooth recovery. It’s great to hear that you are 43 days post-surgery and that your wound is not showing any significant issues. The fact that you have no discharge, and that your stitches have fallen out, is a positive sign of healing.
Regarding the use of the ointment, it is generally advisable to stop using any topical medication once the wound has healed sufficiently and there are no signs of infection or irritation. Since you mentioned that there are no secretions and no bleeding or pain after bowel movements, it is likely safe to discontinue the use of the Neomycin ointment. However, it’s always best to consult your surgeon or healthcare provider for personalized advice, as they are familiar with your specific case and can provide the most accurate recommendations.

The itching and discomfort you are experiencing could be attributed to a few factors. It might be a normal part of the healing process as the tissue continues to recover and possibly shrinks. Itching can also occur as the nerves in the area heal and regenerate. If the ointment you are using contains ingredients that may cause irritation, that could also contribute to the sensation you are feeling.
If you are considering using over-the-counter hemorrhoid ointments, it is important to choose one that is specifically designed for post-surgical care. Some products may contain ingredients that could irritate the area further, so it’s wise to read the labels carefully. Additionally, if you decide to use a new product, it would be prudent to consult with your healthcare provider beforehand to ensure it won’t interfere with your healing process.

In terms of general post-operative care, here are some additional tips to promote healing and comfort:
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to prevent constipation, which can strain the surgical site during bowel movements. Staying hydrated is also essential.

2. Hygiene: Maintain good hygiene in the area. Gentle cleaning with warm water after bowel movements can help keep the area clean without causing irritation.

3. Avoid Straining: If you feel the urge to have a bowel movement, do not strain. If necessary, consider using stool softeners to ease the process.

4. Pain Management: If you experience discomfort, over-the-counter pain relievers can be helpful, but consult your doctor for recommendations tailored to your situation.

5. Follow-Up Appointments: Attend any scheduled follow-up appointments with your surgeon to monitor your recovery and address any concerns.

6. Rest and Activity: While it’s important to stay active, avoid heavy lifting or strenuous activities that could stress the surgical site until you receive clearance from your healthcare provider.

In conclusion, while it seems you are on the right track with your recovery, always prioritize communication with your healthcare provider regarding any changes or concerns. They can provide the best guidance tailored to your specific situation, ensuring a safe and effective recovery process.
