Post-Fistula Surgery Care: Managing Pain and Medication Application - Surgery

Post-fistula surgery

Hello Doctor, I underwent a fistula excision surgery on January 11th.
I am taking sitz baths four times a day, and I experience slightly more pain than usual during the baths, along with some bleeding.
I would like to know if this is normal.
The surgeon prescribed me with gentamicin ointment to apply twice daily, but since I take sitz baths four times a day, it washes off the ointment.
Should I reapply it after each sitz bath?

Zhao, 20~29 year old female. Ask Date: 2024/01/17

Dr. Zhang Shuhao reply Surgery

It is normal to experience slightly more pain and some bleeding during sitz baths before the wound has healed, and this will improve over time.
If sitz baths are performed more frequently, the amount of ointment can be reduced, and it may even be sufficient to apply it only in the morning and evening.

After undergoing fistula surgery, managing pain and medication application is crucial for a smooth recovery. It’s common to experience some discomfort and minor bleeding during the healing process, especially when engaging in activities like sitz baths. Here’s a comprehensive overview of what to expect and how to manage your care post-surgery.

Pain Management
1. Understanding Pain Levels: Post-surgery pain can vary from mild to moderate, especially during activities like sitz baths. It’s normal to feel some discomfort, but if the pain becomes severe or is accompanied by excessive bleeding, you should contact your healthcare provider.

2. Pain Relief Medications: Your doctor may have prescribed pain relief medications. It’s important to take these as directed. Over-the-counter pain relievers like acetaminophen or ibuprofen can also be helpful, but consult your doctor before combining medications.

3. Sitz Baths: These baths are beneficial for promoting healing and reducing discomfort. However, if you notice that the pain increases significantly during or after the bath, you might want to limit the duration or frequency. Ensure the water is warm but not hot, as extreme temperatures can exacerbate pain.

Medication Application
1. Antibiotic Ointment: The use of an antibiotic ointment like Neomycin (欣黴素) is common to prevent infection. If you are taking sitz baths four times a day, it’s likely that the ointment will wash off.
2. Reapplication: It is advisable to reapply the ointment after each sitz bath. This ensures that the surgical site remains protected from infection and promotes healing. Make sure to gently pat the area dry before applying the ointment to avoid irritation.

3. Monitoring the Surgical Site: Keep an eye on the surgical site for any signs of infection, such as increased redness, swelling, or discharge that is yellow or green. If you notice any of these symptoms, contact your healthcare provider immediately.

Additional Care Tips
1. Hygiene: Maintain good hygiene around the surgical area. Clean the area gently with mild soap and water during your daily routine, but avoid scrubbing the site.

2. Diet and Hydration: A balanced diet rich in vitamins and minerals can aid in recovery. Staying hydrated is also essential, as it helps maintain skin elasticity and overall health.

3. Follow-Up Appointments: Attend all scheduled follow-up appointments with your surgeon. These visits are crucial for monitoring your recovery and addressing any concerns you may have.

4. Activity Level: Limit strenuous activities and heavy lifting during the initial recovery phase. Listen to your body and rest when needed.

5. Emotional Support: Surgery can be a stressful experience, and it’s important to address any emotional or psychological concerns. Don’t hesitate to reach out to friends, family, or a mental health professional if you feel overwhelmed.

Conclusion
In summary, experiencing some pain and minor bleeding after fistula surgery is normal, especially during sitz baths. It’s essential to manage your pain effectively and ensure that your antibiotic ointment is applied correctly after each bath. Keep an eye on your surgical site for any signs of infection and maintain open communication with your healthcare provider regarding your recovery. By following these guidelines, you can help ensure a smoother healing process and reduce the risk of complications.
