Blurred vision persists after treatment for iritis?
Hello Doctor, I would like to ask about my condition.
On January 5th, I was diagnosed with conjunctivitis in my right eye.
On January 13th, due to worsening blurred vision, I consulted another ophthalmologist who confirmed it as iritis.
On January 16th, I went to a large hospital for further treatment, where they said the inflammation markers had decreased significantly.
They also tested for HLA-B27, which came back positive.
On January 20th, during a follow-up visit, the inflammation markers continued to drop to very low levels.
However, I am puzzled because my right eye remains blurry.
I had laser surgery for my myopia over a decade ago, and the doctor mentioned that my left eye has a myopia of 50-75, while my right eye is 150-175.
Before I developed iritis, I did not experience such severe blurriness.
My right eye feels as if it is filled with tears, creating a foggy sensation, making it difficult to see clearly.
I plan to return for a follow-up on February 3rd.
What additional tests should I ask the doctor to perform? This foggy sensation is very distressing! I would appreciate your guidance.
Thank you!

Persistent blurriness after uveitis treatment can be a frustrating and concerning issue. It’s important to understand that while inflammation may be decreasing, the recovery of vision can take time and may be influenced by several factors. Here’s a comprehensive overview of your situation and some recommendations for your next steps.
Understanding Uveitis and Its Effects on Vision
Uveitis, particularly iritis (inflammation of the iris), can lead to various symptoms, including blurred vision, light sensitivity, and pain. The inflammation can cause swelling and changes in the eye's structure, which may affect how light is focused on the retina. Even after the inflammation has decreased, residual effects can linger, leading to persistent blurriness.
Factors Contributing to Blurriness
1. Residual Inflammation: Even if the inflammatory markers are decreasing, there may still be some residual inflammation affecting your vision. This can lead to symptoms like blurriness or a watery sensation in the eye.
2. Corneal Edema: Inflammation can sometimes lead to swelling of the cornea (corneal edema), which can cause blurriness. This condition may resolve as the inflammation subsides.
3. Changes in Eye Pressure: Uveitis can sometimes lead to changes in intraocular pressure, which can also affect vision. Monitoring eye pressure is crucial during treatment.
4. Cataract Formation: Long-term use of corticosteroids, which are often prescribed for uveitis, can lead to cataract formation. If you have been on steroids, this could be a potential cause of your blurriness.
5. Underlying Conditions: The positive HLA-B27 test indicates a potential association with autoimmune conditions, which can complicate the clinical picture and may lead to additional ocular issues.
Next Steps and Recommendations
1. Follow-Up Appointment: Since you have a follow-up appointment scheduled for February 3rd, it’s essential to discuss your ongoing symptoms with your ophthalmologist. Be sure to mention the specific nature of your blurriness and any other symptoms you may be experiencing.
2. Additional Tests: Your doctor may consider conducting additional tests, such as:
- Optical Coherence Tomography (OCT): This non-invasive imaging test can provide detailed images of the retina and help assess any swelling or other changes.
- Slit Lamp Examination: A thorough examination can help identify any residual inflammation or other ocular issues.
- Intraocular Pressure Measurement: Monitoring your eye pressure can help rule out glaucoma or other pressure-related issues.
3. Medication Review: Discuss your current medications with your doctor. If you are still experiencing significant symptoms, your doctor may adjust your treatment plan, which could include changing the frequency or type of eye drops you are using.
4. Lifestyle Adjustments: While you continue your treatment, consider minimizing eye strain by taking regular breaks from screens and reading. Use artificial tears to keep your eyes lubricated, especially if you experience dryness.
5. Monitor Symptoms: Keep a diary of your symptoms, noting any changes in vision, pain, or light sensitivity. This information can be valuable for your doctor in assessing your condition.
Conclusion
While it can be distressing to experience persistent blurriness after treatment for uveitis, it’s important to remember that recovery can take time. By maintaining open communication with your healthcare provider and following their recommendations, you can work towards improving your symptoms. Remember that your vision may gradually improve as inflammation continues to resolve, but it’s crucial to monitor your condition closely and seek further evaluation if necessary.
