Consultation on Trichomoniasis Infection Issues

Hello Doctor, about two years ago I was diagnosed with a Trichomonas vaginalis infection, and at that time, the doctor prescribed "Johnson" Metronidazole tablets for seven days.
After completing the treatment, I did not return for a follow-up.
Recently, my girlfriend (who has been the same partner since two years ago) has been experiencing frequent urination.
I would like to ask the doctor three questions: 1.
Since I have heard that Trichomonas may not always be accurately detected, can we go directly to a family medicine clinic (as I hope my girlfriend and I can be treated together) and explain the situation to the doctor, asking for a prescription for Trichomonas medication for both of us? 2.
Can the medication for Trichomonas effectively treat both oral and vaginal infections? 3.
Since I took Metronidazole two years ago, if a prescription is needed, should I ask the doctor to use a different medication to avoid the risk of drug resistance? Thank you very much for taking the time to respond, Doctor.

Trichomoniasis, caused by the protozoan parasite Trichomonas vaginalis, is a common sexually transmitted infection (STI) that can affect both men and women. It is important to address your concerns regarding treatment and management of this infection, especially considering your history and your girlfriend's current symptoms.

1. Consultation and Testing: It is indeed advisable for both you and your girlfriend to visit a healthcare provider together. You can explain your history of trichomoniasis and your girlfriend's symptoms, such as frequent urination, which may or may not be related to the infection. While it is true that trichomoniasis can sometimes be difficult to detect through standard testing, a healthcare provider can evaluate your symptoms and may decide to treat both of you empirically, especially if there is a high suspicion of the infection. This approach is often taken to prevent reinfection and to ensure both partners are treated simultaneously.

2. Treatment of Oral and Vaginal Trichomoniasis: The standard treatment for trichomoniasis is metronidazole or tinidazole, which are effective against the parasite in both the genital and oral areas. If your girlfriend has been diagnosed with trichomoniasis, it is important to ensure that both of you receive treatment to eliminate the infection. Metronidazole is typically administered as a single dose or over a course of several days, depending on the severity of the infection. It is effective in treating trichomoniasis in both the vagina and the oral cavity, as the medication works systemically to eradicate the parasite.

3. Antibiotic Resistance Concerns: Regarding your previous treatment with metronidazole (the brand name "强生" refers to Johnson & Johnson, which markets metronidazole under various names), it is a valid concern to consider the potential for antibiotic resistance. However, metronidazole remains the first-line treatment for trichomoniasis, and resistance is relatively uncommon. If you are retreated, your healthcare provider may still prescribe metronidazole, as it is generally effective. If there are concerns about resistance or if you have had recurrent infections, your doctor may consider alternative treatments or dosages. It is crucial to follow your healthcare provider's recommendations and complete the full course of treatment to minimize the risk of resistance.

In summary, it is essential for both you and your girlfriend to seek medical attention together. Discuss your previous infection and her current symptoms openly with your healthcare provider. They can provide appropriate testing and treatment options. Remember to complete the prescribed course of medication and follow up with your doctor if symptoms persist or recur. Additionally, practicing safe sex and regular STI screenings can help prevent future infections.
