Treatment for preputial edema?
Hello doctor, a couple of days ago I masturbated quite vigorously, and that day I experienced some pain in my left pelvic and thigh joint.
I had a fever that night, which subsided the next day, but my foreskin has swollen up.
It has been swollen for two days now, and the skin on my glans is a bit dry.
I visited a nearby dermatology clinic for treatment, and they prescribed me doxepin ointment (which I found out is used for treating atopic dermatitis).
It feels slightly cool and has a mild stinging sensation, but the swelling has not yet gone down.
Should I visit a urology clinic, or should I just continue using this medication for now? Thank you for your response.

When managing preputial edema, it is essential to understand the underlying causes and when to seek further medical attention. Based on your description, it seems that you have experienced some discomfort and swelling in the prepuce (foreskin) following vigorous masturbation, accompanied by fever and localized pain in the pelvic region. You have also consulted a dermatologist who prescribed doxepin ointment, which is typically used for conditions like atopic dermatitis.
First, let's address the symptoms you are experiencing. Preputial edema can result from various factors, including irritation, infection, or trauma. In your case, the vigorous activity may have caused irritation or microtrauma to the sensitive skin of the prepuce, leading to swelling and discomfort. The presence of fever could indicate a possible infection, which is a concern that should not be overlooked.
The doxepin ointment you were prescribed is an antihistamine that can help alleviate itching and inflammation associated with skin conditions. However, if the swelling persists or worsens, or if you develop additional symptoms such as increased pain, discharge, or persistent fever, it is crucial to seek further evaluation from a urologist. A urologist specializes in conditions affecting the urinary tract and male reproductive system, and they can provide a more comprehensive assessment of your symptoms.
Here are some specific signs that would warrant a visit to a urologist:
1. Persistent Swelling: If the preputial edema does not begin to resolve within a few days of using the ointment, it may indicate an underlying issue that requires further investigation.
2. Increased Pain: If you experience worsening pain in the pelvic area or during urination, this could suggest an infection or other complications that need medical attention.
3. Fever: The initial fever you experienced could be a sign of infection. If you develop a fever again or have chills, it is essential to seek medical care promptly.
4. Discharge: Any unusual discharge from the penis or prepuce should be evaluated by a healthcare professional, as it may indicate an infection.
5. Difficulty Urinating: If you experience any difficulty or pain while urinating, this is another reason to consult a urologist.
In summary, while it is reasonable to start with the treatment prescribed by your dermatologist, you should closely monitor your symptoms. If there is no improvement or if you notice any concerning signs, do not hesitate to make an appointment with a urologist. They can perform a thorough examination, possibly including tests such as a urinalysis or cultures, to determine the cause of your symptoms and provide appropriate treatment. Remember, early intervention can prevent complications and promote a quicker recovery.
