A small hard lump?

Hello Dr.
Li,
I would like to ask you about something: about a month ago, I suddenly noticed a hard, round lump near the bone below my middle finger on my palm.
It is somewhat painful when pressed, but it does not affect my daily activities.
What could be the possible cause of this? Does it require treatment? Thank you!
Monica

Dr. Li Junde reply Surgery

Ms.
Monica: Pain and a hard lump on the middle finger may be a normal sesamoid bone or a tumor arising from the bone, tendon, or subcutaneous tissue.
If there is a change in size or an increase in pain, please visit an orthopedic clinic for evaluation.
Thank you.

Hello Monica,
Thank you for reaching out with your question regarding the painful bump you’ve noticed on your hand. Based on your description, it sounds like you may be experiencing a condition known as a ganglion cyst, which is a common benign lump that typically forms near joints or tendons, particularly in the hands and wrists. These cysts are filled with a jelly-like fluid and can vary in size. They often appear suddenly and may cause discomfort when pressure is applied, but they usually do not interfere with daily activities.

Another possibility could be a condition known as a mucous cyst, which often occurs at the distal interphalangeal joint (the joint closest to the fingertip) and is associated with osteoarthritis. These cysts can also be painful and may feel firm to the touch.

In some cases, the bump could be a result of a more serious condition, such as a bone spur or a soft tissue tumor, but these are less common. Given that you mentioned the bump is painful when pressed but does not affect your daily activities, it is likely benign. However, it’s essential to monitor the bump for any changes in size, shape, or pain level.

As for treatment options, many ganglion cysts do not require treatment unless they cause significant pain or discomfort. If the cyst is bothersome, there are several approaches you can consider:
1. Observation: If the bump is not causing significant pain or affecting your daily life, you may choose to simply monitor it. Many ganglion cysts will resolve on their own over time.

2. Immobilization: Sometimes, wearing a splint or brace can help reduce the size of the cyst by limiting movement and allowing the area to rest.

3. Aspiration: A healthcare provider can use a needle to drain the fluid from the cyst. This procedure can provide immediate relief, but cysts can recur.

4. Corticosteroid Injection: Injecting a corticosteroid into the cyst may help reduce inflammation and pain, although this does not guarantee that the cyst will not return.

5. Surgery: If the cyst is persistent, painful, or affecting your function, surgical removal may be considered. This is typically a last resort after other treatments have failed.

It’s important to consult with a healthcare professional, such as an orthopedic specialist or a hand surgeon, who can perform a physical examination and possibly imaging studies (like an ultrasound or MRI) to confirm the diagnosis and rule out other conditions.

In summary, while the bump you are experiencing is likely benign, it’s essential to keep an eye on it and consult with a healthcare provider if you have any concerns or if the symptoms worsen. Early intervention can help prevent complications and ensure proper management of the condition.

Wishing you the best in your health journey!
Best regards.
