Postoperative Modified Ferguson with Ligasure?

Hello, Doctor.
It has been ten days since my complete excision of internal and external hemorrhoids, and I am experiencing minimal pain now.
I continue to rinse with warm water after using the restroom at home, and when I am outside, I use wet wipes.
I also take warm sitz baths at least twice a day and apply the ointment prescribed by my surgeon, which is a skin softening ointment.
However, sometimes I feel a mild stinging sensation around the anal area, as if the skin is raw and has come into contact with something irritating.
I also notice a slight feeling of moisture from discharge.
The condition fluctuates; some days I do not experience this sensation at all, but other days, shortly after getting up for work, I feel this stinging sensation.
I would like to know if this is due to the skin around the anal area not healing yet or if there might be another issue.
Thank you for taking the time to respond amidst your busy schedule.

Dr. Ke Fangxu reply Surgery

The wound after hemorrhoidectomy typically takes about 2 to 4 weeks to fully heal.
Mild discomfort or stinging during this period is normal.
It is recommended to take warm sitz baths frequently or apply topical ointments to help alleviate the irritation at the wound site.

Post-operative care following a surgical procedure, such as the Modified Ferguson with Ligasure surgery, is crucial for ensuring proper healing and minimizing complications. Based on your description, it seems you are experiencing some discomfort and irritation around the anal area ten days post-surgery, which is not uncommon. Here’s a comprehensive overview of what you might expect during your recovery, along with some recommendations for managing your symptoms.

Understanding Post-Operative Symptoms
1. Pain and Discomfort: It is normal to experience some level of discomfort after surgery, especially in the initial days. The fact that your pain has decreased significantly is a positive sign. However, the intermittent sharp or stinging sensations you describe could be due to several factors:
- Healing Process: The skin and tissues around the surgical site are still healing. As they regenerate, it’s common to experience sensations like tingling or mild pain.

- Irritation: The sensation of "broken skin" you mentioned could be due to irritation from friction, moisture, or even the application of topical ointments. The skin in that area is sensitive, and any external factors can exacerbate discomfort.

2. Discharge: The presence of a moist feeling or discharge can also be typical post-surgery. This could be due to:
- Surgical Drainage: It’s possible that some fluid is still being expelled from the surgical site as part of the healing process.

- Infection: While less likely if you are following proper care protocols, any unusual discharge, especially if it has a foul odor or is accompanied by increased redness or swelling, should be evaluated by your physician.

Recommendations for Care
1. Hygiene: Continue with your current hygiene practices, including warm water rinses and sitz baths. These can help soothe the area and promote healing. Ensure that you are drying the area gently after washing to prevent moisture buildup, which can lead to irritation.

2. Topical Treatments: Using the ointment prescribed by your surgeon is essential. If you notice increased irritation, consider discussing with your doctor whether a different topical treatment might be more suitable. Some patients find that using a barrier cream can help protect sensitive skin.

3. Monitoring Symptoms: Keep a close eye on your symptoms. If the stinging sensation worsens, or if you notice any signs of infection (increased redness, swelling, or discharge), contact your healthcare provider promptly.

4. Activity Modification: If certain activities or positions exacerbate your discomfort, consider modifying them until you feel more comfortable. Avoiding prolonged sitting or activities that put pressure on the area may help alleviate some of the irritation.

5. Follow-Up Appointments: Ensure you attend any scheduled follow-up appointments with your surgeon. These visits are crucial for monitoring your recovery and addressing any concerns you may have.

When to Seek Help
If your symptoms persist beyond a reasonable timeframe or if you experience severe pain, significant swelling, or any signs of infection, it is essential to reach out to your healthcare provider. They may need to evaluate the surgical site to ensure that healing is progressing as expected and to rule out any complications.

In summary, while some discomfort and irritation can be expected after your surgery, it’s important to monitor your symptoms closely and maintain good hygiene practices. If you have any concerns or if your symptoms change, do not hesitate to consult your healthcare provider for further guidance. Your recovery is a priority, and addressing any issues early can help ensure a smooth healing process.
