Perineum, masturbation, and condom issues?

1.
During masturbation, the perineum becomes very hard with any exertion.
If I do not exert myself, I can last 10-20 minutes, but as soon as I exert, I feel an immediate urge to ejaculate, and even with an erection, exerting the perineum can lead to immediate ejaculation.
I would like to know if frequent masturbation could lead to perineal weakness or erectile dysfunction, or if there are other reasons for this? (Masturbation frequency is once a day.)
2.
After achieving an erection during masturbation or sex, if I change positions and do not continue stimulation for too long, I easily lose my erection.
What can I do to improve this? (I have very short sleep, usually going to bed between 2-4 AM, and I do not exercise often.
How significantly does this affect me? If I improve my sleep schedule and start exercising, how long might it take to see recovery?)
3.
After foreplay, I am very hard, but as soon as I put on a condom, I lose my erection.
If I do manage to penetrate, I ejaculate very quickly.
What could be the reason for this? P.S.
I am a premature baby and have hypospadias (1-1.5 cm).
I may need to shake a few times to clean myself after urinating.
Could this impact my sexual function? Thank you, doctor, for your answers.

Hello:
1.
During masturbation, the perineal area becomes very hard with any exertion.
If I don't exert myself, I can last 10-20 minutes, but as soon as I exert, I immediately feel the urge to ejaculate, and even with an erection, exerting the perineal area can lead to immediate ejaculation.
I would like to know if frequent masturbation can lead to perineal weakness or erectile dysfunction, or if there are other reasons? (Masturbation frequency is once a day) —> This should not make a difference.
2.
After getting an erection during masturbation or sex, if I change positions and do not continue stimulation for too long, I easily lose my erection.
What can I do to improve this? (I have very short sleep, usually sleeping from 2 to 4 AM, and I do not exercise often.
Is this severely affecting me? If I rely on improving my routine and exercise, how long might it take to recover?) —> It may be necessary to pay more attention to lifestyle, diet, and exercise.
Losing an erection without stimulation may be a habit formed from previous experiences.
3.
After foreplay, I am very hard, but as soon as I put on a condom, I lose my erection.
If I do manage to penetrate, I ejaculate very quickly.
What could be the reason for this? —> This is likely a habit formed from DIY experiences.

P.S.
I was a premature baby and have hypospadias (1-1.5 cm).
I may need to shake a few times to clean myself properly when using the restroom.
Could this have an impact on my sexual function? —> It should not.

Wishing you good health.

Understanding perineal issues, particularly in relation to masturbation, erection problems, and the use of condoms, can be complex and multifaceted. Let's address your questions one by one, incorporating relevant medical knowledge.

1. Frequent Masturbation and Perineal Weakness: You mentioned that during masturbation, your perineal area becomes very hard when you exert force, and you experience a quick ejaculation. Frequent masturbation, in itself, is generally not harmful and does not typically lead to perineal weakness or erectile dysfunction. However, if you are experiencing a rapid ejaculation or difficulty controlling your ejaculation, it might be related to the way you are stimulating yourself rather than the frequency of masturbation. The pelvic floor muscles, which include the perineal muscles, can become tense or overactive with frequent or forceful stimulation, potentially leading to a quick ejaculation response. It may be beneficial to practice relaxation techniques or pelvic floor exercises (like Kegel exercises) to help improve control over ejaculation.

2. Erection Loss During Position Changes: The issue of losing an erection when changing positions or after a period of inactivity is common and can be influenced by several factors. Sleep deprivation, as you mentioned, can significantly affect sexual performance and libido. Lack of sleep can lead to hormonal imbalances, increased stress, and fatigue, all of which can contribute to erectile difficulties. Regular exercise can improve blood circulation, enhance mood, and boost testosterone levels, which may help with erectile function. Improving your sleep schedule and incorporating physical activity into your routine can take several weeks to show noticeable improvements in sexual health, but consistency is key. Aim for at least 30 minutes of moderate exercise most days of the week and try to establish a regular sleep pattern.

3. Erection Loss with Condom Use: The phenomenon of losing an erection when putting on a condom is not uncommon and can be attributed to several factors. Anxiety about performance, the sensation of the condom, or even the interruption of foreplay can contribute to this issue. It’s important to find a condom that feels comfortable for you, as some may be thicker or have textures that can affect sensitivity. Additionally, ensuring that both partners are engaged and comfortable during the process can help maintain arousal. If you find that using condoms consistently leads to erection loss, consider discussing alternative forms of contraception with your partner or a healthcare provider.

Regarding your history as a premature infant with hypospadias, it is possible that this condition could have some impact on your sexual function, particularly if there were any surgical interventions or complications during development. However, many individuals with a history of hypospadias lead normal sexual lives. If you have concerns about how this might affect your sexual health, it would be advisable to consult a urologist or a healthcare provider specializing in sexual health.

In summary, while frequent masturbation is unlikely to cause perineal weakness or erectile dysfunction, factors such as sleep deprivation, anxiety, and physical conditioning can significantly impact sexual performance. Addressing lifestyle factors, practicing relaxation techniques, and ensuring comfort during sexual activities can help improve your experiences. If these issues persist, consider seeking professional advice for a more tailored approach to your sexual health.
