Painful foreskin

Hello doctor, I have phimosis, and recently, over the past month, I've noticed an increase in the white discharge on the glans and foreskin while showering.
Sometimes when I retract the foreskin, I experience a stinging sensation, and occasionally I feel pain after masturbation.
I make it a point to retract the foreskin and wash away the white discharge every day during my shower, and I also rinse the glans thoroughly, usually getting it quite clean.
Is this excessive cleaning? I've heard that over-cleaning can be harmful.
Should I not be cleaning the white discharge on the glans? Thank you, doctor.

Hello: The white discharge should be rinsed off and dried.
It is recommended to use only water for rinsing, and not to use body wash.
It is important to keep the area dry.
If engaging in sexual activity, please practice safe sex.
If the issue persists, seek medical evaluation.
Wishing you good health.

More Info

Phimosis, or the inability to retract the foreskin over the glans (head) of the penis, can lead to various symptoms, including discomfort, pain, and hygiene issues. The presence of white discharge or smegma, which is a combination of dead skin cells, oils, and other bodily fluids, is common in individuals with phimosis. It is essential to maintain proper hygiene to prevent infections and other complications, but it is equally important to avoid over-cleaning, which can lead to irritation.

Understanding the Symptoms
1. Pain and Discomfort: The pain you experience, especially during activities like masturbation or when retracting the foreskin, can be attributed to inflammation or irritation of the sensitive skin of the glans and foreskin. This can occur if there is an accumulation of smegma or if the skin is overly dry or irritated from excessive cleaning.

2. Increased Smegma Production: The increased amount of white discharge you are noticing could be due to a few factors, including the natural buildup of smegma, which is more pronounced in individuals with phimosis. If the foreskin cannot be retracted easily, it can trap moisture and debris, leading to a higher accumulation of smegma.

Hygiene Tips
1. Gentle Cleaning: While it is important to clean the area, it is crucial to do so gently. Use warm water and mild soap to clean the glans and the inside of the foreskin. Avoid harsh soaps or scrubbing too hard, as this can irritate the sensitive skin and exacerbate any discomfort.

2. Frequency of Cleaning: Cleaning the area daily is generally sufficient. Over-cleaning can strip the skin of its natural oils, leading to dryness and irritation. If you notice that your skin is becoming red or irritated, consider reducing the frequency of washing.

3. Drying: After washing, ensure that the area is dried thoroughly but gently. Moisture can promote bacterial growth, so keeping the area dry is essential.

4. Moisturizing: If you experience dryness or irritation, consider using a gentle, fragrance-free moisturizer. This can help soothe the skin and prevent further irritation.

When to Seek Medical Attention
If you continue to experience pain, discomfort, or an increase in symptoms despite following proper hygiene practices, it is advisable to consult a healthcare professional. They can assess your condition and may recommend treatments such as:
- Topical Steroids: In some cases, a doctor may prescribe a topical steroid cream to reduce inflammation and help with retraction of the foreskin.

- Surgical Options: If phimosis is severe and causing significant discomfort or hygiene issues, surgical options such as circumcision or preputioplasty may be considered.

Conclusion
Maintaining proper hygiene is essential for managing phimosis and preventing complications. However, it is equally important to avoid over-cleaning, which can lead to irritation and discomfort. If you have concerns about your symptoms or hygiene practices, do not hesitate to reach out to a healthcare provider for personalized advice and treatment options. They can provide you with the best guidance tailored to your specific situation.
