Pregnancy test questions?

Hello doctor, I have a question.
Last month, my menstrual cycle was regular at 28 days.
I had sexual intercourse on February 3, February 4, and from February 12 to 17, with ejaculation inside on the 12th.
After that, I cleaned myself and took a shower, then immediately went to buy emergency contraception.
My period was expected on the 26th but did not arrive.
I took a pregnancy test on March 3, which was positive.
The day before, I had taken headache medication.
Am I pregnant? When would you recommend I take another test?

ting, 20~29 year old female. Ask Date: 2024/03/07

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
The effectiveness of emergency contraception is not 100%, with a failure rate of about 3%, but it is still considered a remedial method.
It is advisable to check for pregnancy if your period is more than a week late.
Thank you! Wishing you peace and health.

When it comes to confirming a pregnancy, timing and the method of testing are crucial. Based on your situation, it seems you have had regular menstrual cycles, with your last period starting on January 29. You mentioned having unprotected intercourse on February 12, which is significant as it falls within your fertile window, especially if you have a typical 28-day cycle.
You noted that your period was due on February 26 but did not arrive, which is often the first indicator of a potential pregnancy. You conducted a pregnancy test on March 3, which showed a positive result. This is a promising sign, as home pregnancy tests are designed to detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining.
To address your specific questions:
1. Is it likely that you are pregnant? Given that you had unprotected intercourse during your fertile window and your period was late, a positive pregnancy test on March 3 is a strong indication that you may be pregnant.
2. Should you retest? It is generally recommended to wait a few days to a week after the first positive test to retest, especially if you want to confirm the result. This is because hCG levels double approximately every 48 to 72 hours in early pregnancy. If you test too early, the levels may not be high enough to be detected, leading to a false negative. Since your last test was on March 3, you could consider testing again around March 6 or 7 for a more definitive result.

3. What about the headache medication? Taking headache medication should not affect the results of a pregnancy test. However, if you are pregnant, it's always a good idea to consult with a healthcare provider regarding any medications you are taking, as some may not be recommended during pregnancy.

4. Next steps: If you continue to receive positive results, it would be wise to schedule an appointment with your healthcare provider. They can perform a blood test, which is more sensitive than home tests, and can also provide you with an ultrasound to confirm the pregnancy and check for a viable gestational sac.

In summary, based on the information provided, it appears that you may be pregnant, especially with the positive test result. Retesting in a few days will help confirm this. If you have any concerns or experience unusual symptoms, such as severe pain or heavy bleeding, seek medical attention promptly. Congratulations on this potential new chapter, and best wishes for your health moving forward!
