Right lower abdominal tenderness?

My menstrual cycle is relatively regular, usually occurring every 26 to 38 days.
Recently, I've been experiencing tenderness in the lower right abdomen (the family medicine doctor mentioned it is near the uterus and ovaries), and occasionally it will hurt on its own.
I would like to ask the doctor if this is typical ovulation pain or could it be endometriosis?

Understanding right lower abdominal pain can be quite complex, especially when considering conditions like ovulation pain and endometriosis. Based on your description, it seems you are experiencing localized pain in the right lower abdomen, which could indeed be related to either ovulation or a more serious condition like endometriosis.

Ovulation pain, also known as mittelschmerz, typically occurs around the midpoint of your menstrual cycle, which aligns with your reported cycle length of 26 to 38 days. This pain is usually unilateral, meaning it can occur on one side of the abdomen, and is often described as a sharp or cramping sensation. It can last from a few minutes to a few hours and is generally not accompanied by other significant symptoms. The pain is thought to be caused by the release of an egg from the ovary, which can irritate the surrounding tissues.

On the other hand, endometriosis is a condition where tissue similar to the lining inside the uterus grows outside of it, often affecting the ovaries, fallopian tubes, and the tissue lining the pelvis. This condition can cause significant pain, especially during menstruation, but it can also lead to chronic pelvic pain that may occur at any time during the menstrual cycle. The pain associated with endometriosis can be more severe than typical ovulation pain and may be accompanied by other symptoms such as heavy menstrual bleeding, pain during intercourse, and gastrointestinal issues.

Given that you mentioned the pain is sometimes spontaneous and can be quite intense, it raises the question of whether it could be related to endometriosis. The presence of other symptoms, such as changes in your menstrual cycle, abnormal bleeding, or gastrointestinal symptoms, could further support this possibility.
It is important to note that while ovulation pain is generally benign, endometriosis can lead to complications such as infertility and the formation of adhesions, which can cause further pain and complications. Therefore, if you are experiencing significant discomfort or if the pain persists or worsens, it is crucial to seek further evaluation from a healthcare provider.
A thorough examination, including a pelvic exam and possibly imaging studies like an ultrasound or MRI, can help determine the underlying cause of your pain. In some cases, a laparoscopy may be necessary to visualize the pelvic organs directly and confirm a diagnosis of endometriosis.

In summary, while your symptoms could be indicative of normal ovulation pain, the possibility of endometriosis should not be overlooked, especially if the pain is severe or accompanied by other concerning symptoms. Consulting with a gynecologist for a comprehensive evaluation would be the best course of action to ensure an accurate diagnosis and appropriate management.
