Male urinary issues
Hello Dr.
Du, about three weeks ago, I visited a urology clinic due to itching of the foreskin and a burning sensation during urination.
I was prescribed Mecon ointment and took the antibiotic Cephalexin for a week, which helped me recover.
However, recently I've occasionally felt a slight burning sensation when urinating again.
A few days ago, during some DIY activities, I was quite excited and ended up doing it twice.
After the second time, I noticed a small blood clot in the semen.
I would like to ask: 1.
Is it possible that the occasional burning sensation during urination (not every time) indicates a recurrent infection? 2.
I've been experiencing frequent lower back pain; could this be related to a urinary infection? 3.
Is it normal to have a small blood clot in the semen? (This has never happened before.) If not, could it be related to the previous inflammation, or could it be another issue? Thank you for your time, Dr.
Du!

Hello:
1.
Is it possible to have a recurrent infection if I occasionally feel a burning sensation when urinating (not every time)?
- Yes, it is possible, but the severity is usually mild.
It is recommended to perform a urinalysis to confirm.
If it is just a one-time issue, it is mostly not an infection.
2.
I have been frequently experiencing lower back pain.
Could it be related to a urinary infection?
- If the pain persists or if other symptoms occur, it may be related.
3.
Is it normal to have a small blood clot in semen? (This has never happened before.)
- It is clinically common and generally not a major concern, but you should still seek medical attention.
Wishing you good health.

Hello! Thank you for your detailed questions regarding your urological concerns. It’s important to address these issues comprehensively, as they can significantly impact your health and well-being. Let’s break down your questions one by one.
1. Occasional Burning Sensation During Urination: The intermittent burning sensation you’re experiencing could indicate a few possibilities. While it’s great that you’ve seen improvement after treatment with antibiotics, the recurrence of symptoms may suggest a few things. It could be a sign of a recurrent urinary tract infection (UTI) or inflammation of the urethra, possibly due to irritation or residual infection. It’s also possible that the initial infection wasn’t completely eradicated. If this sensation persists or worsens, it would be wise to consult your urologist for further evaluation, which may include urine tests or cultures to identify any underlying infections.
2. Lower Back Pain: Your concern about lower back pain is valid, as it can sometimes be associated with urological issues. Infections in the urinary tract can lead to discomfort in the lower back, particularly if the infection ascends to the kidneys, resulting in pyelonephritis. However, back pain can also stem from various other causes, including muscle strain, especially if you’ve been engaging in physical activities or lifting heavy objects. If the back pain is persistent or accompanied by other symptoms such as fever or increased urinary urgency, it’s essential to seek medical attention to rule out any serious conditions.
3. Presence of Blood Clots in Semen: The appearance of a blood clot in your semen is concerning and should not be taken lightly. While it’s not uncommon for men to occasionally notice blood in their semen (a condition known as hematospermia), it is usually not considered normal, especially if it’s a new occurrence for you. This could be related to several factors, including inflammation of the prostate (prostatitis), infection, or even trauma to the reproductive organs. Given your recent history of inflammation and treatment, it’s plausible that this could be related to your previous condition. However, it’s crucial to have this evaluated by a healthcare professional to rule out any serious underlying issues.
Additional Considerations:
- Chronic Prostatitis: Given your history of chronic prostatitis, it’s essential to maintain regular follow-ups with your urologist. Chronic prostatitis can lead to recurrent symptoms and may require ongoing management, including lifestyle modifications, dietary changes, and possibly long-term medication.
- Lifestyle Factors: Your lifestyle, including hydration, diet, and physical activity, can significantly impact your urological health. Staying well-hydrated can help dilute urine and reduce irritation. Additionally, avoiding irritants such as caffeine, alcohol, and spicy foods may help alleviate symptoms.
- Sexual Activity and Recovery: Engaging in sexual activity, including masturbation, during recovery from a urological infection can sometimes exacerbate symptoms. It’s generally advisable to wait until you’re symptom-free and have consulted with your doctor before resuming sexual activity.
In conclusion, while some of your symptoms may be related to previous infections or inflammation, it’s crucial to have a thorough evaluation by a urologist to ensure proper diagnosis and treatment. Don’t hesitate to reach out to your healthcare provider with any new or worsening symptoms. Your health is paramount, and addressing these concerns promptly can help prevent further complications. Thank you for your questions, and I wish you the best in your recovery!
