Chest tightness, insomnia?

Hello, doctor.
I would like to ask what kind of diseases or heart conditions could cause left chest tightness and insomnia.

1.
I am currently having difficulty falling asleep and rely on Eszopiclone and antiarrhythmic medication to help me sleep.
After falling asleep, I consistently wake up around 2-3 AM to urinate, and when I lie back down, I feel a tightness in my left chest, which is uncomfortable.
Sometimes, my hands and feet feel numb, and I can't fall back asleep until morning.
This also happens when I sit with my eyes closed for a while, especially when I'm about to fall asleep or after lying in bed for some time.
During the day, I occasionally feel tightness in my left chest.
(I have hypertension and was initially taking Candesartan, but my doctor switched me to antiarrhythmic medication, yet my symptoms have not improved.)
2.
I have undergone an electrocardiogram, a 24-hour Holter monitor, an echocardiogram, and a stress test (which showed positive results at stages 3 and 4).
A coronary angiogram indicated that one artery is 10% blocked.
3.
In October, I was able to fall asleep, but as soon as I drifted off, my heart would wake me up.
This led to the onset of insomnia.
After undergoing tests, I had a coronary angiogram in December, and since then, I wake up every morning to urinate, and when I lie back down, I feel discomfort and tightness in my left chest (sometimes accompanied by a sensation of blood swirling around and numbness in my hands and feet).

Dr. He Dongjin reply Cardiology

Hello, the cardiac catheterization showed that only one blood vessel is 10% blocked, which means there are no significant issues.
Considering your symptoms, it is suggested that you consult a psychosomatic medicine specialist.
Best wishes from Changhua Hospital.

Understanding left chest pain and insomnia can be complex, especially when considering the interplay between physical health and psychological factors. Based on the symptoms you've described, it seems that there are multiple potential contributors to your discomfort, including anxiety, sleep disturbances, and possibly underlying cardiovascular issues.

1. Chest Pain and Insomnia: Left chest pain can arise from various sources, including musculoskeletal issues, gastrointestinal problems, and cardiac conditions. Given your history of hypertension and previous cardiac evaluations, it's essential to consider both physical and psychological factors. Anxiety and stress can significantly impact your perception of pain and lead to symptoms such as chest tightness or discomfort. This is often exacerbated during periods of heightened anxiety, which may explain why you experience more discomfort when trying to relax or fall asleep.

2. Sleep Disturbances: The fact that you are relying on medications like Eszopiclone (艾斯樂) and heart rhythm medications to aid sleep suggests that your insomnia may be linked to both anxiety and possibly your cardiovascular health. The sensation of waking up suddenly, feeling your heart racing, and experiencing chest discomfort upon lying down could indicate a heightened state of anxiety or panic, which is not uncommon in individuals with a history of anxiety disorders.
3. Physical Symptoms: The left chest discomfort you experience, along with the tingling in your hands and feet, could be related to anxiety-induced hyperventilation, which can cause temporary changes in blood flow and lead to sensations of numbness or tingling. Additionally, the feeling of blood "circulating erratically" may also be a manifestation of anxiety rather than a direct cardiovascular issue.

4. Cardiovascular Health: Your previous tests, including echocardiograms and stress tests, have indicated only minor issues, such as a 10% blockage in one artery. While this is something to monitor, it is not typically considered significant enough to cause the symptoms you describe. However, it is crucial to maintain regular follow-ups with your healthcare provider to monitor your heart health, especially given your hypertension.

5. Management Strategies:
- Lifestyle Modifications: Incorporating stress-reduction techniques such as mindfulness, meditation, or yoga may help alleviate anxiety and improve sleep quality. Regular physical activity can also be beneficial for both mental and cardiovascular health.

- Sleep Hygiene: Establishing a consistent sleep routine, creating a comfortable sleep environment, and avoiding stimulants before bedtime can enhance your ability to fall asleep and stay asleep.

- Medication Review: It may be worthwhile to discuss your current medications with your healthcare provider to ensure they are appropriate for your symptoms and to explore alternatives if necessary.

6. When to Seek Help: If your symptoms worsen or if you experience new symptoms such as severe chest pain, shortness of breath, or significant changes in your heart rhythm, it is crucial to seek immediate medical attention.
In conclusion, while your symptoms may be concerning, they appear to be influenced by a combination of anxiety, sleep disturbances, and possibly minor cardiovascular issues. A comprehensive approach that includes both medical management and lifestyle changes may help improve your overall well-being. Always consult with your healthcare provider for personalized advice and treatment options tailored to your specific situation.
