The sensitivity of the glans has decreased?

Hello, I have been experiencing some concerns lately, and I'm not sure if it's related to my previous spinal surgery.

1.
Recently, I have noticed a decrease in sensitivity of the glans, and during ejaculation, the sensation is not as good as it used to be (I only masturbate once a week).
The volume of semen has also decreased.
Why is this happening?
2.
Normally, after urinating, I tend to use toilet paper to wipe, but shortly afterward, I notice a clear, slippery fluid leaking out.
At that moment, I wasn't watching any adult content or engaging in any stimulating activities, and my penis was not erect.
Why is this occurring?
I have discreetly mentioned this to a neurosurgeon and underwent examinations, but nothing significant was found.
Now, I am unsure who else to ask.
Please help me, thank you.

Decreased sensitivity in the glans (the head of the penis) can be a concerning issue for many individuals, and it can arise from various causes. Based on your description, it seems that you are experiencing a combination of decreased sensitivity, changes in ejaculation, and unexpected discharge. Let's break down these concerns and explore potential causes and considerations.

1. Decreased Sensitivity and Changes in Ejaculation:
The decrease in sensitivity in the glans can be attributed to several factors. One common cause is nerve damage or changes in nerve function, which can occur after spinal surgery. The spinal cord is responsible for transmitting signals between the brain and the rest of the body, including the genital area. If there has been any trauma or surgical intervention in the spinal region, it could potentially affect the nerves that innervate the penis, leading to decreased sensitivity.

Additionally, hormonal changes can also play a role. Testosterone levels naturally decline with age, and lower levels can affect sexual function, including sensitivity and ejaculation. If you have experienced significant changes in your sexual health, it may be worthwhile to discuss hormone levels with your healthcare provider.

The reduction in the volume of ejaculate can also be linked to various factors, including hydration levels, frequency of ejaculation, and even psychological factors such as stress or anxiety. If you are only engaging in sexual activity infrequently (once a week), this can also lead to changes in the volume of semen produced.

2. Unexpected Discharge:
The clear, slippery discharge you are experiencing after urination could be a few things. It is not uncommon for men to experience pre-ejaculatory fluid, which can be released even when not sexually aroused. This fluid is produced by the Cowper's glands and can sometimes be mistaken for other types of discharge. If this discharge is not accompanied by any pain, itching, or unusual odor, it may not be a cause for concern.

However, if you notice any changes in the characteristics of the discharge, such as color, consistency, or if it becomes associated with discomfort, it would be advisable to seek further evaluation. Conditions such as prostatitis or urethritis can lead to abnormal discharge and may require treatment.

3. Psychological Factors:
It is essential to consider the psychological aspects of sexual health. Anxiety, stress, and depression can significantly impact sexual function and sensitivity. If you are feeling anxious about your sexual health or have concerns about your body image or performance, this could contribute to decreased sensitivity and changes in sexual function. Engaging in open conversations with a mental health professional or a sex therapist can provide valuable support.

4. Consultation with Specialists:
Since you have already consulted with a neurosurgeon and undergone examinations without finding any significant issues, it may be beneficial to seek a second opinion or consult with a urologist who specializes in male sexual health. They can perform a thorough evaluation, including hormone testing, nerve function assessments, and other diagnostic tests to determine the underlying cause of your symptoms.

In summary, decreased sensitivity in the glans and changes in ejaculation can arise from a combination of physical, hormonal, and psychological factors. It is crucial to maintain open communication with your healthcare providers and seek further evaluation if symptoms persist or worsen. Addressing these concerns holistically can lead to improved sexual health and overall well-being.
