HPV vaccine and antibiotics?

Hello Dr.
Hsiao, due to prostatitis, I have recently started taking antibiotics (Lisacef 500mg and OA18-Cephradine 500mg).
Next week is scheduled for my second dose of the HPV nine-valent vaccine.
Can I still receive it as planned? If not, what is the maximum interval that must be maintained between the first and second doses of the HPV vaccine? Thank you.

Dear Capybara:
1.
According to the recommendations of Taiwan's Centers for Disease Control, the schedule for administering the HPV nine-valent vaccine consists of three doses: one at 0 months, one at 1 to 2 months, and one at 6 months.
In cases of severe infections, immunocompromised status, incomplete vaccination, pregnancy, allergies, or other special issues, vaccination may need to be delayed or even discontinued.
2.
Since you are suffering from prostatitis and must take antibiotics, if it is acute, the course of medication may last about a week; therefore, you can receive the vaccine as soon as you complete the medication.
If it is chronic, you may need to take antibiotics for a month or even longer; in this case, you should consult your attending physician about when to administer the second dose.
3.
From a theoretical standpoint, the established vaccination schedule is based on experimental data that yields the best outcomes.
In practice, however, there are many variables that may arise, and it is essential for the physician to make appropriate decisions based on the specific circumstances of the patient at that time.
4.
Personally, I believe that as long as the prostatitis is adequately controlled—meaning no fever, no significant discomfort, or contraindications—the second dose should be able to be administered.
After all, the HPV vaccine is an inactivated vaccine and does not pose a risk of HPV infection from vaccination.

When it comes to receiving the HPV vaccine while on antibiotics, the good news is that antibiotics do not interfere with the effectiveness of the HPV vaccine. The HPV vaccine is designed to stimulate the immune system to produce antibodies against the virus, and antibiotics primarily target bacterial infections. Therefore, taking antibiotics for a bacterial infection, such as prostatitis in your case, should not affect your ability to receive the HPV vaccine.

In your situation, since you are scheduled to receive the second dose of the HPV nine-valent vaccine (Gardasil 9), you can proceed with the vaccination as planned, even while taking antibiotics. It is always a good practice to inform your healthcare provider about any medications you are taking, including antibiotics, but there is no contraindication for receiving the HPV vaccine while on these medications.

Regarding the timing of the doses, the HPV vaccine is typically administered in a series of two or three doses, depending on the age at which the vaccination series is started. For individuals aged 15 and older, the standard schedule is three doses: the second dose is given 1-2 months after the first dose, and the third dose is given 6 months after the first dose. If you are unable to receive the second dose on the scheduled date, it is generally recommended to get it as soon as possible. There is flexibility in the timing of the doses, and it is better to receive the vaccine later than to miss it altogether.

If you miss the second dose, you should aim to get it within a reasonable timeframe, ideally within a few months. However, if it has been longer than the recommended intervals, you should consult your healthcare provider for guidance on how to proceed. They may recommend restarting the series or continuing with the next dose, depending on how much time has elapsed since your first dose.

In summary, you can receive the HPV vaccine while taking antibiotics without any concerns about its effectiveness. Just ensure that you keep your healthcare provider informed about your medications and follow up on the timing of your vaccine doses to ensure optimal protection against HPV. Vaccination is a crucial step in preventing HPV-related diseases, including cervical cancer and other HPV-associated cancers, so staying on track with your vaccination schedule is important for your health.
