Questions related to gastric ulcer biopsy?
Last week, I had an upper gastrointestinal endoscopy, and the doctor mentioned that I have a mild gastric ulcer, but he said a biopsy is needed for confirmation.
I would like to ask if it is common for patients with gastric ulcers to require a biopsy, or is it because my case is particularly severe that a biopsy is necessary?

Dr. Chen Shidian reply Gastroenterology and Hepatology
A convenient biopsy will be performed to obtain approximately 0.5 cm of gastric mucosal tissue for pathological examination.

When it comes to the management of stomach ulcers, the decision to perform a biopsy is often guided by several factors, including the characteristics of the ulcer, the patient's symptoms, and the potential risk of malignancy. In general, not all patients with stomach ulcers require a biopsy; however, there are specific circumstances where it is deemed necessary.
Biopsies are typically performed during an endoscopy when there are concerns about the nature of the ulcer. For instance, if the ulcer appears atypical, is larger than expected, or if there are accompanying symptoms such as significant weight loss, gastrointestinal bleeding, or a family history of gastric cancer, a biopsy is warranted. The biopsy helps to rule out malignancy and to assess for the presence of Helicobacter pylori, a bacterium that can contribute to ulcer formation and is also associated with gastric cancer.
In your case, the doctor recommended a biopsy likely to ensure that the ulcer is not cancerous and to check for any underlying conditions that could complicate your treatment. This is a standard precautionary measure, especially in patients who present with ulcers, as it provides valuable information about the ulcer's nature and guides further management.
Regarding the severity of your condition, the need for a biopsy does not necessarily indicate that your ulcer is particularly severe. It is more about ensuring a comprehensive evaluation of your gastrointestinal health. In many cases, ulcers can be benign, but the biopsy serves as a safeguard to confirm this.
If the biopsy results show chronic gastritis or intestinal metaplasia, as indicated in some of the previous reports, it may suggest a need for regular monitoring. Intestinal metaplasia, in particular, is considered a precancerous condition, and while it does not mean you have cancer, it does require follow-up to monitor for any changes over time.
In terms of lifestyle and dietary changes, these can significantly impact the healing of stomach ulcers. Patients are often advised to avoid irritants such as NSAIDs, alcohol, and smoking, as these can exacerbate ulcer symptoms and hinder healing. A diet rich in fruits, vegetables, and whole grains, along with adequate hydration, can promote gastrointestinal health. Some studies suggest that probiotics may also be beneficial in maintaining gut health and potentially aiding in ulcer healing.
In conclusion, while not all patients with stomach ulcers require biopsies, your doctor’s recommendation is a prudent step to ensure your health and safety. Regular follow-up and monitoring, along with lifestyle modifications, can play a crucial role in managing your condition effectively. If you have further concerns or questions about your treatment plan, it is always best to discuss them directly with your healthcare provider, who can provide personalized advice based on your specific situation.
