Medication consultation
Dear Doctor,
I visited a Traditional Chinese Medicine practitioner due to gastroesophageal reflux disease, and my current symptoms are: 1.
Sour taste in the mouth and occasional bloating.
2.
A thick white coating on my tongue with a vertical line in the middle.
3.
Difficulty falling asleep at night, and if I wake up in the middle of the night, I find it hard to fall back asleep.
4.
There are two dental blisters on my gums.
The doctor prescribed me the following medications: 6 grams of Banxia Xiexin Decoction, 3 grams of Chaihu Shugan Decoction, and 3 grams of Yunu Decoction.
The doctor mentioned that Yunu Decoction is meant to help eliminate the blisters on my gums!
I would like to ask: 1.
If the blisters on my gums disappear during the course of the medication, should I still finish taking the entire week's worth of medicine? Is Yunu Decoction considered a very cold medicine? I am a bit concerned because it contains gypsum.
2.
Or does Yunu Decoction also have therapeutic effects on my other symptoms, so I can continue taking it without worry?
Thank you for your response!

Dr. Gao Peiyi reply Traditional Chinese Medicine
1.
If my dental abscess has resolved during the course of medication, should I still continue taking the medication for another week? Is the Yu Nu Decoction a very cooling medicine? I'm a bit concerned because it contains gypsum! Yes, you can continue taking it.
The main formula is Banxia Xiexin Decoction at 6 grams, while Yu Nu Decoction is only 3 grams, which is a low proportion; your current condition seems to indicate some excess heat.
2.
Or does the Yu Nu Decoction actually have therapeutic effects on my other symptoms, so I can continue taking it with peace of mind? Yes, you can continue taking it.
The Yu Nu Decoction helps clear stomach heat and nourish yin, which may be beneficial for sleep.

Understanding Traditional Chinese Medicine (TCM) for Gastroesophageal Reflux (GERD) involves a holistic approach that considers not only the physical symptoms but also the underlying imbalances in the body. In your case, you are experiencing symptoms such as sour taste in the mouth, bloating, thick white tongue coating, difficulty sleeping, and dental issues. These symptoms suggest a possible disharmony in your digestive system, which TCM aims to address through herbal remedies and lifestyle adjustments.
1. Herbal Treatment and Its Effects: The prescription you received includes Banxia Xiexin Decoction (半夏瀉心湯), Chaihu Shugan Decoction (柴胡疏肝湯), and Yunu Decoction (玉女煎). Each of these formulas has specific functions:
- Banxia Xiexin Decoction is traditionally used to harmonize the stomach and alleviate symptoms such as nausea and bloating. It can help in cases of stomach Qi stagnation.
- Chaihu Shugan Decoction is aimed at soothing the liver and relieving stress, which can be a contributing factor to digestive issues.
- Yunu Decoction is often used to nourish the stomach and clear heat, which may also address the dental issues you mentioned.
2. Addressing Your Concerns:
- If the dental sores (牙泡) have resolved during the course of treatment, it is generally advisable to complete the full course of medication as prescribed. This ensures that the underlying condition is fully addressed and helps prevent recurrence. Stopping the medication prematurely might lead to a resurgence of symptoms.
- Regarding your concern about Yunu Decoction being "cold" due to its ingredients, including gypsum (石膏), it is important to understand that TCM categorizes herbs based on their energetic properties. While some ingredients may be considered cold, they are balanced by others in the formula. The overall effect of the decoction is designed to harmonize and support your body's needs. If you have specific concerns about the cold nature of the herbs, it is best to discuss this with your TCM practitioner, who can provide personalized advice based on your constitution and current symptoms.
3. Holistic Approach: TCM emphasizes the importance of lifestyle and dietary choices in managing GERD. It is advisable to avoid spicy, greasy, and acidic foods, as well as to maintain a calm and stress-free environment. Incorporating gentle exercises like Tai Chi or Qi Gong can also help improve digestion and reduce stress.
4. Monitoring Symptoms: Keep track of your symptoms as you continue the herbal treatment. If you notice any adverse reactions or if your symptoms worsen, it is crucial to communicate this with your TCM practitioner. They may need to adjust your prescription or explore other treatment options.
5. Long-term Management: TCM can be a valuable long-term strategy for managing GERD and related symptoms. Regular follow-ups with your practitioner can help adjust your treatment plan as your condition evolves. Additionally, consider integrating practices such as acupuncture, which may provide further relief from symptoms and promote overall well-being.
In conclusion, TCM offers a comprehensive approach to managing GERD by addressing both symptoms and underlying imbalances. It is essential to maintain open communication with your healthcare provider and to follow their guidance closely. By doing so, you can work towards achieving better digestive health and overall wellness.
