Your EBV Test Results: Managing Anxiety and Health Concerns - Oncology

Thank you, doctor, for taking the time to respond, which has helped ease my anxiety and allowed me to sleep better?

Hello Doctor, I am very anxious and would like to ask a few questions.
I have not had any cough, but I constantly feel like I have phlegm.
I visited several clinics, and the doctors prescribed medication for mild inflammation.
After seeing three different doctors, they only prescribed medication for a month.
Suddenly, I noticed that the color of the phlegm seemed to have blood in it, as it appeared dark red or brown.
I went to You-Ting Clinic in Hsinchu for an endoscopy, and they found a protrusion in my right nasal cavity, while the left side was clear.
The doctor mentioned it looked more like benign lymphoid tissue and referred me to National Taiwan University Hospital in Hsinchu.
However, the health insurance app indicated nasopharyngeal malignancy.
When I arrived at NTU, the doctor also said that such irregularities are not uncommon, and they performed a biopsy and blood tests.
The health insurance app at NTU later indicated a benign nasopharyngeal tumor.
Then I saw my blood test report:
EBNA IgG 134.07 ——————————————
Test Name: EB Virus Nuclear Antigen Antibody Test
Test Item: EBNA IgG
Result: 134.07 units *Reference Value [<9 (*unit: NTU).
(Note: Negative: <9 NTU; Equivocal: 9-11 NTU; Positive: >11 NTU)]
EBV-VCA IgA 0.59 —————————————————
Test Name: EB Virus Capsid Immunoglobulin G, M, A (IFA Method)
Test Item: EBV-VCA IgA
Result: 0.59 units *Reference Value [<9 (*unit: NTU).
(Note: Negative: <9 NTU; Equivocal: 9-11 NTU; Positive: >11 NTU)]
I would like to ask: Is the EBNA IgG 134.07 positive and significantly above the standard? Is it dangerous? I am still waiting for the pathology report, but I am very anxious, unable to eat or sleep.
Can you please help me with this? Thank you.

Dr. Chen Yunfang reply Oncology

Hello: Yes, it is very dangerous.
The normal EBV DNA levels in healthy individuals are close to zero, which likely indicates a possibility of nasopharyngeal carcinoma.
Please wait for the biopsy pathology results; until the pathology report is available, everything remains highly suspicious and cannot be definitively diagnosed.
Thank you!

Understanding your EBV test results and managing anxiety regarding potential health concerns can be quite challenging, especially when faced with conflicting information and symptoms. Let’s break down your situation step by step.

Firstly, your EBNA IgG result of 134.07 is indeed significantly above the normal reference range, which is less than 9 NTU. This indicates that you have been exposed to the Epstein-Barr Virus (EBV) at some point in your life, as IgG antibodies typically develop after an infection. However, it’s crucial to understand that a positive EBNA IgG does not directly indicate an active infection or a diagnosis of nasopharyngeal carcinoma (NPC). The presence of these antibodies suggests past exposure rather than current disease.

The EBV-VCA IgA result of 0.59 is within the normal range, indicating that there is no current active EBV infection. In the context of EBV, the IgA antibodies are often associated with acute infection, and their absence can be reassuring.
Regarding your symptoms, such as the presence of blood-tinged phlegm and the finding of a mass in your nasal cavity, it’s understandable to feel anxious. The fact that the initial assessment suggested a benign lymphoid hyperplasia and that the biopsy is pending is a positive sign. Benign conditions can often mimic more serious diseases, and many individuals experience similar symptoms without having cancer.

It’s also important to note that the healthcare system can sometimes create confusion with terminology. The mention of “nasopharyngeal malignancy” in your health app could be a precautionary note rather than a definitive diagnosis. The final diagnosis will depend on the histopathological examination of the biopsy, which is the gold standard for determining the nature of the tissue.

In terms of managing your anxiety, it’s essential to focus on what you can control. Here are some strategies:
1. Stay Informed but Not Overwhelmed: While it’s good to seek information, try to limit your exposure to potentially distressing content online. Stick to reputable medical sources or consult your healthcare provider for clarity.

2. Communicate with Your Doctor: If you have concerns about your symptoms or test results, don’t hesitate to reach out to your healthcare provider. They can provide context and reassurance based on your specific situation.

3. Mindfulness and Relaxation Techniques: Engaging in mindfulness practices, such as meditation or deep-breathing exercises, can help reduce anxiety. Physical activity, even a short walk, can also improve your mood.

4. Support Systems: Talk to friends or family about your feelings. Sometimes, sharing your worries can alleviate some of the burden.

5. Professional Help: If your anxiety becomes overwhelming, consider seeking support from a mental health professional. They can provide coping strategies tailored to your needs.

In conclusion, while your EBV test results indicate past exposure, they do not confirm an active infection or malignancy. The upcoming biopsy results will provide more definitive information. In the meantime, focus on managing your anxiety and maintaining open communication with your healthcare team. Remember, you are not alone in this, and there are resources available to help you navigate through your concerns.
