Gastroenteritis issues?

Hello, doctor.
I've been experiencing diarrhea for the past few days, with only small amounts of watery stools, and I've been going several times a day.
However, I saw a doctor yesterday, and they diagnosed me with gastroenteritis.
Last night around 3 AM, I had stomach pain that kept me from sleeping; I had to sit up to feel a bit better.
I would like to know if this is also a symptom of gastroenteritis.
Do I need to see a doctor again?

Miss Zheng, 20~29 year old female. Ask Date: 2024/05/07

Dr. Cai Anshun reply Gastroenterology and Hepatology

Hello: If there is no improvement, it is recommended to return for further examination.
If you are awakened by pain in the middle of the night, it is advisable to have a lighter dinner with smaller portions and to avoid eating two hours before bedtime.

More Info

Gastroenteritis, commonly referred to as stomach flu, is an inflammation of the stomach and intestines that can lead to a variety of symptoms, including diarrhea, abdominal pain, nausea, vomiting, and fever. Your recent experience of watery diarrhea multiple times a day, accompanied by significant abdominal pain, particularly at night, aligns with the typical presentation of gastroenteritis.

The symptoms you describe, such as the inability to lie down comfortably due to pain and the need to sit up to alleviate discomfort, are indeed consistent with gastroenteritis. This condition can be caused by various factors, including viral infections (like norovirus or rotavirus), bacterial infections (such as E. coli or Salmonella), or even parasitic infections. In some cases, gastroenteritis can also be triggered by food intolerances or allergies, as well as stress or anxiety.

Given that you have already seen a doctor who diagnosed you with gastroenteritis, it is essential to monitor your symptoms closely. If your symptoms are mild and you are able to stay hydrated, you may not need to return to the doctor immediately. However, there are specific signs and symptoms that warrant a follow-up visit or immediate medical attention:
1. Severe Dehydration: If you notice signs of dehydration, such as excessive thirst, dry mouth, little or no urination, dizziness, or confusion, you should seek medical help. Dehydration can occur quickly with diarrhea, especially if you are not able to keep fluids down.

2. Persistent or Worsening Symptoms: If your abdominal pain becomes more severe, or if you experience high fever (over 101°F or 38.3°C), blood in your stool, or persistent vomiting, you should return to the doctor.

3. Duration of Symptoms: If your diarrhea lasts more than a couple of days without improvement, or if you are unable to keep fluids down for more than 24 hours, it is advisable to seek medical attention.

4. Underlying Health Conditions: If you have any underlying health conditions (such as diabetes, kidney disease, or a weakened immune system), you should consult your doctor sooner rather than later.

In terms of management, staying hydrated is crucial. Oral rehydration solutions (ORS) can be beneficial, as they contain electrolytes that help replenish lost fluids. Avoiding dairy products, caffeine, and high-fat or spicy foods can also help minimize gastrointestinal irritation during recovery.

As for your question about whether this is a typical manifestation of gastroenteritis, the answer is yes. The abdominal pain, particularly if it is crampy and associated with diarrhea, is a common symptom. The fact that you experienced significant pain at night, which improved when sitting up, suggests that your gastrointestinal tract is quite irritated, which is characteristic of gastroenteritis.

In conclusion, while your symptoms are consistent with gastroenteritis, it is essential to remain vigilant for any warning signs that may indicate a more severe condition. If you experience any of the aforementioned symptoms or if your condition does not improve, do not hesitate to return to your healthcare provider for further evaluation and management.
