Long-Term Effects of Head Trauma: Post-Concussion Symptoms - Neurosurgery

Post-concussion syndrome?

Hello, Doctor! I would like to ask about my mother's condition.
Five years ago, she was driving and hit the left side of her head against the car window.
At the time, there was no bleeding, but there was swelling, and she applied ice.
She experienced dizziness for a short period and did not vomit.
She assessed the situation herself and thought it wasn't serious, so she only rested for a few days and did not go to the hospital.
The area that was hit occasionally hurts during that month, but for the past five years, she has experienced sudden pain in that area for about a few minutes at a time.
I am wondering if this could be similar to post-concussion syndrome lasting this long, or if the impact might have triggered some underlying head issue.
Thank you for your response, Doctor!

Chen, 20~29 year old female. Ask Date: 2024/05/10

Dr. Lai Zhaokang reply Neurosurgery

Hello: In theory, it is rare to have such long-lasting sequelae (five years), but it is still possible.
If you are concerned, take her to the hospital to see a neurosurgeon for evaluation.
Wishing you all the best.

Head trauma can have a variety of long-term effects, especially if the injury was significant enough to cause symptoms such as pain, dizziness, or cognitive changes. In your mother's case, the incident five years ago where she hit her head against the car window may have resulted in what is commonly referred to as post-concussion syndrome (PCS). This condition can occur after a concussion and is characterized by a range of symptoms that can persist for weeks, months, or even years after the initial injury.

Post-concussion syndrome can manifest in various ways, including headaches, dizziness, difficulty concentrating, memory problems, and mood changes. The fact that your mother experiences intermittent pain at the site of the injury suggests that there may be lingering effects from the trauma. While it is not uncommon for symptoms to improve over time, some individuals may continue to experience issues long after the initial injury, particularly if the injury was not properly evaluated or treated.

The symptoms your mother is experiencing could be attributed to several factors. First, the initial impact may have caused damage to the soft tissues, nerves, or even the underlying structures of the skull. This could lead to chronic pain or sensitivity in the affected area. Additionally, the brain itself may have experienced a mild traumatic injury that did not present with immediate severe symptoms but has resulted in ongoing issues.

It is important to note that the brain is a complex organ, and injuries can have far-reaching effects that are not always immediately apparent. For instance, even if your mother did not experience severe symptoms at the time of the injury, she may still be dealing with the consequences of the trauma. Factors such as stress, lack of sleep, and other health issues can exacerbate these symptoms, making them feel more pronounced.

Given the duration and nature of your mother's symptoms, it would be advisable for her to seek a thorough evaluation from a healthcare professional, preferably a neurologist or a specialist in brain injuries. They may recommend imaging studies, such as an MRI or CT scan, to assess any structural changes in the brain or surrounding tissues. Additionally, a comprehensive neurological examination can help determine if there are any cognitive or functional impairments that need to be addressed.

In terms of management, treatment for post-concussion syndrome often involves a multidisciplinary approach. This may include physical therapy, cognitive rehabilitation, and medication to manage pain or other symptoms. Lifestyle modifications, such as stress management techniques, regular exercise, and a healthy diet, can also play a significant role in recovery.

In conclusion, while it is not uncommon for individuals to experience lingering symptoms following a head injury, it is crucial to seek professional medical advice to rule out any serious underlying issues and to develop an appropriate treatment plan. Early intervention can often lead to better outcomes and improved quality of life.
