Delayed Periods: Pregnancy or Stress? - Obstetrics and Gynecology

Pregnancy probability

Hello Doctor, my partner's menstrual cycle is regular.
We had intercourse on the last day of her period, April 13th, and we used a condom throughout.
However, her period is now 5 days late.
She mentioned that she has been under a lot of stress lately.
Could you please advise whether this is a sign of pregnancy or if it could be due to her stress causing the delay in her period? Additionally, during her luteal phase, she experienced brown discharge and bright red blood.
Is this a sign of pregnancy?

Hello, you should not be pregnant.
Wait for another week, and if your period still hasn't arrived, I recommend seeing a gynecologist.
Wishing you good health.

When it comes to understanding delayed periods, it’s essential to consider various factors, including the possibility of pregnancy and the impact of stress. In your partner's case, she experienced a delay of five days in her menstrual cycle, which can be concerning, especially when there has been recent sexual activity. However, it’s important to analyze the situation carefully.

First and foremost, the use of condoms during intercourse significantly reduces the risk of pregnancy, provided that the condom was used correctly and did not break. Since your partner's last menstrual period ended on April 13, and you had intercourse on that day, the timing of ovulation is crucial. Typically, ovulation occurs about 14 days before the next expected period, but this can vary from person to person. If her cycle is regular, ovulation would likely have occurred around the end of April, which means that the timing of intercourse could have been close to her fertile window.

The presence of brown discharge and fresh red blood during the luteal phase (the time between ovulation and the start of the next period) can be indicative of several things. Brown discharge often signifies old blood, which may occur due to hormonal fluctuations or even implantation bleeding if pregnancy has occurred. However, it is also common for women to experience spotting or changes in discharge due to stress or hormonal imbalances, especially if they are under significant emotional or physical stress.

Stress is a well-documented factor that can lead to menstrual irregularities. High levels of stress can affect the hypothalamus, which regulates the hormones responsible for the menstrual cycle. This disruption can lead to delayed ovulation or even anovulation (the absence of ovulation), resulting in a delayed period. Given that your partner has been experiencing considerable stress, it is plausible that this could be contributing to her delayed menstruation.

To determine whether pregnancy is a possibility, it is advisable for your partner to take a home pregnancy test. These tests are most reliable when taken after a missed period, as they measure the hormone hCG (human chorionic gonadotropin), which is produced during pregnancy. If the test is negative and her period does not arrive within a week, it may be wise to consult a healthcare provider for further evaluation.

In summary, while there is a possibility of pregnancy due to the timing of intercourse, the stress your partner is experiencing could also be a significant factor in her delayed period. The combination of these elements makes it essential to monitor her symptoms closely and consider taking a pregnancy test for clarity. If the test is negative and her period remains absent, seeking medical advice would be prudent to rule out any underlying issues and to discuss stress management strategies.
