Hysteroscopy post-operative care?

Hello, I had a hysteroscopic polypectomy on May 27th.
On the day of the surgery and the following day, I experienced very little brown discharge.
By the third and fourth days, the bleeding increased and included some red blood, similar to the beginning of my menstrual cycle (enough to use a panty liner).
I also feel a slight bloating in my abdomen.
Is this normal?

Chung, 30~39 year old female. Ask Date: 2024/05/30

Dr. Liu Fuping reply Obstetrics and Gynecology

It is normal to experience a small amount of vaginal bleeding or light yellow discharge after hysteroscopic surgery.
However, if there is heavy bleeding, the discharge has an unusual odor, or if abdominal pain or fever occurs, you should seek medical attention promptly.

Post-operative recommendations: Rest in bed for one day after returning home; generally, you can resume daily activities after 1 to 2 days.
For the first 7 days, please take showers, maintain cleanliness in the genital area, and avoid vaginal douching and sexual intercourse.
For one month, avoid lifting heavy objects and engaging in vigorous exercise.
If the physician has prescribed medication, please take it as directed.

More Info

After undergoing a hysteroscopy for polyp removal on May 27, it is common to experience some bleeding and abdominal discomfort during the recovery period. The symptoms you describe, including a small amount of brownish discharge on the day of the surgery and the following day, followed by an increase in bleeding that resembles the start of a menstrual cycle, are generally considered normal post-operative experiences.

What to Expect After Hysteroscopy
1. Bleeding: It is typical to have light bleeding or spotting after a hysteroscopy. This can vary from person to person, but it often starts as brownish discharge, which is old blood, and can progress to brighter red blood as the healing process continues. The amount of bleeding can fluctuate, and it may resemble a light menstrual period for a few days to a week post-surgery.

2. Abdominal Discomfort: Mild cramping or a feeling of fullness in the abdomen is also common after the procedure. This discomfort is usually due to the manipulation of the uterus during the surgery and the healing process. If the discomfort is mild and manageable, it is generally not a cause for concern.

3. Recovery Timeline: Most patients can expect to return to their normal activities within a few days after the procedure. However, it is advisable to avoid strenuous activities, heavy lifting, and sexual intercourse for at least a week or as directed by your healthcare provider.

4. Signs of Complications: While some bleeding and discomfort are normal, certain symptoms should prompt you to contact your healthcare provider. These include:
- Heavy bleeding (soaking through a pad in an hour or less)
- Severe abdominal pain that does not improve with over-the-counter pain relief
- Fever or chills
- Foul-smelling discharge
- Dizziness or fainting
5. Follow-Up Care: It is essential to attend any scheduled follow-up appointments with your healthcare provider to ensure proper healing and to discuss any concerns you may have. Your provider may perform an ultrasound or other evaluations to check the status of your uterus and ensure that there are no complications.

Conclusion
In summary, the symptoms you are experiencing after your hysteroscopy are generally within the range of normal post-operative recovery. However, it is crucial to monitor your symptoms and reach out to your healthcare provider if you notice any concerning changes or if your symptoms worsen. Each individual's recovery process can vary, so maintaining open communication with your healthcare team is key to ensuring a smooth recovery.
