Menstrual pain began after engaging in sexual activity?
Hello Doctor: Is it normal to suddenly experience menstrual pain after starting to engage in sexual activity, even though I never had it before? From the time I was in junior high until now, I have never experienced menstrual pain.
However, after trying digital penetration with my boyfriend in March of this year, I started to experience menstrual cramps, with discomfort in my lower abdomen lasting 1-3 days.
It's not severe enough to require pain medication, but it is uncomfortable.
Additionally, I would like to ask about the HPV vaccine: when is it most effective to receive it before engaging in sexual activity? How long after completing the vaccination series is it recommended to have sexual intercourse? If I haven't received the third dose yet, does that mean the level of protection is reduced?
Miss Lin, 20~29 year old female. Ask Date: 2024/06/03
Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, theoretically it should not be related.
If you are concerned, it is advisable to see a gynecologist for an examination.
It is best to wait until after completing all three doses before engaging in sexual activity.
Wishing you good health.

More Info
Experiencing menstrual cramps after initiating sexual activity can be concerning, especially if you previously did not have any discomfort during your menstrual cycle. It is important to understand that various factors can contribute to this change in your body’s response.
Firstly, sexual activity, including activities like fingering, can stimulate the pelvic area and may lead to increased sensitivity in the uterus and surrounding tissues. This stimulation can sometimes result in cramping or discomfort during your menstrual cycle. The hormonal changes that accompany sexual activity can also affect your menstrual cycle, potentially leading to cramps. Stress and anxiety related to new sexual experiences can further exacerbate physical symptoms, including menstrual cramps.
If you have started to experience cramps that are mild and manageable, it may not be a cause for alarm. However, if the cramps become severe or are accompanied by other symptoms such as heavy bleeding, unusual discharge, or significant pain, it would be wise to consult a healthcare provider to rule out any underlying conditions such as endometriosis or pelvic inflammatory disease.
Regarding your question about the HPV vaccine, it is recommended to complete the full vaccination series for optimal protection against human papillomavirus (HPV), which can lead to cervical cancer and other health issues. The HPV vaccine is most effective when all doses are completed before becoming sexually active. The vaccine typically consists of three doses, and it is advisable to wait until you have received all three doses before engaging in sexual activity to ensure maximum protection. If you have not completed the series, it does not mean you are unprotected, but the level of protection may not be as high as it would be after the full vaccination.
In summary, experiencing menstrual cramps after starting sexual activity can be normal due to various physiological and psychological factors. If the discomfort persists or worsens, seeking medical advice is recommended. Additionally, completing the HPV vaccination series before becoming sexually active is the best way to ensure you have the highest level of protection. If you have any further concerns or questions, do not hesitate to reach out to a healthcare professional for personalized advice.
