Postoperative care after rubber band ligation for hemorrhoids?
Hello Doctor: I underwent rubber band ligation for internal hemorrhoids on May 24.
After it prematurely loosened on May 26, an external hemorrhoid appeared near the anus.
During my follow-up visit on May 27, the doctor re-tied it (approximately three centimeters from the anal opening).
Today, June 3, I have noticed yellow discharge from the anus; is this normal? Additionally, I still experience pain after bowel movements; is that also normal? Will the swelling and pain from the external hemorrhoid gradually subside? It has been painful for two weeks now (the pain has slowly decreased, but it still feels uncomfortable).
Thank you!

Dr. Zhang Shuhao reply Surgery
After the ligation of internal hemorrhoids, there may be yellow discharge while the wound is still healing, but severe pain is uncommon.
It is recommended to continue warm sitz baths, and doctors usually prescribe medication to relieve symptoms.
If there is fever or severe swelling, pain, or bleeding, it is necessary to return for an immediate evaluation.

More Info
After undergoing rubber band ligation for hemorrhoids, it is not uncommon to experience a range of post-procedure symptoms, including pain, swelling, and discharge. Let's address your concerns in detail.
Normal Post-Procedure Symptoms
1. Pain and Discomfort: It is typical to experience pain after rubber band ligation, especially if there was a previous external hemorrhoid or if the ligation was performed on an internal hemorrhoid. The pain can vary in intensity and may last for a few days to a couple of weeks. The fact that you mentioned the pain is gradually decreasing is a positive sign, indicating that your body is healing.
2. Yellow Discharge: The presence of yellow discharge from the anal area can be concerning, but it is often a normal part of the healing process. This discharge may be a combination of mucus and some blood, especially if there has been irritation or inflammation in the area. However, if the discharge has a foul odor or is accompanied by fever, increased pain, or other systemic symptoms, it would be advisable to consult your healthcare provider, as these could indicate an infection.
3. Swelling of External Hemorrhoids: The swelling of the external hemorrhoid you described can occur after the procedure. This is often due to inflammation and irritation from the ligation process. In most cases, this swelling will gradually subside as the area heals. Applying warm sitz baths can help alleviate discomfort and promote healing.
Management and Care
- Pain Management: Over-the-counter pain relievers such as acetaminophen or ibuprofen can help manage pain. However, always follow the dosage instructions and consult your doctor if you have any concerns about medication interactions or side effects.
- Hygiene: Keeping the anal area clean is crucial. Gentle cleansing with warm water after bowel movements can help reduce irritation. Avoid using harsh soaps or wipes that may cause further irritation.
- Dietary Considerations: To ease bowel movements and reduce straining, consider increasing your fiber intake through fruits, vegetables, and whole grains. Staying well-hydrated is also essential.
- Follow-Up: Since you have already had a follow-up appointment where the doctor re-tied the rubber band, it is important to continue monitoring your symptoms. If the pain persists beyond a few weeks or worsens, or if you notice any signs of infection (such as fever, increased swelling, or pus), you should seek medical attention promptly.
Conclusion
In summary, while some discomfort, discharge, and swelling are normal after rubber band ligation for hemorrhoids, it is essential to monitor your symptoms closely. The gradual reduction in pain is encouraging, and the yellow discharge may be part of the healing process. However, do not hesitate to reach out to your healthcare provider if you have any concerns or if your symptoms do not improve as expected. Your comfort and recovery are paramount, and your doctor can provide tailored advice based on your specific situation.
