Discomfort in the left epididymis?


Hello, doctor.
A year ago, I visited due to difficulty urinating and took antibiotics for a month without improvement.
The doctor diagnosed me with pelvic muscle tension.
I was prescribed muscle relaxants, which slightly improved my urination, but if I stop taking the medication, I revert to having difficulty urinating (feeling of incomplete emptying).
Recently, I've noticed occasional swelling and discomfort on the left side of the epididymis, and upon examination, it is indeed larger than the right side, possibly indicating chronic epididymitis or varicocele.
Although it does not affect my daily life, I am concerned about future fertility.
Will this cause permanent infertility? Thank you.
