Semen, 40 days post-preputial surgery?
Dear Dr.
Du,
I underwent a laser circumcision procedure on May 8 due to phimosis, and the wound has healed.
During this period, I completely abstained from sexual activity and did not engage in any DIY until yesterday (June 16), when I first attempted DIY without any abnormal symptoms.
However, today (June 17), during another DIY attempt, I experienced a burning sensation at the base, and after ejaculation, there was a brief burning sensation on the glans, which appeared noticeably red and swollen.
The initial ejaculate was of normal color, but subsequent discharge contained blood, appearing bright red.
I do not have any other significant pain symptoms, and the sutured wound from the circumcision has not opened or bled.
My urine is clear of blood, and I have no fever or other symptoms.
I do not have any chronic illnesses, injuries, sexual partners, or engage in any sexual activities, and I do not smoke, drink alcohol, or chew betel nut.
I would like to ask the doctor:
1.
Is this condition (hematospermia after a long period of abstinence, such as over a month) common?
2.
Will the hematospermia resolve on its own? If so, how long might it take?
3.
Is it necessary to seek medical attention?
4.
If a visit is not required, are there any precautions I should take in my daily life? For example, should I continue abstaining? If so, for how many days? Or can I maintain a normal frequency of ejaculation without abstaining?
Thank you.

Hello:
1.
Is this condition (the appearance of hematospermia after a long period of abstinence [such as over a month]) common? --- It can be observed, but it is not very common.
2.
Will the hematospermia resolve on its own? If so, how long does it typically take? --- It usually takes about 2-3 weeks.
3.
Is a medical consultation necessary? --- It is advisable to seek medical evaluation.
4.
If a consultation is not necessary, are there any lifestyle considerations to keep in mind? --- Maintain a light diet, minimize spicy foods, and drink plenty of water.
For example, should one continue abstaining? / How many more days of abstinence? / Is it okay to maintain a normal frequency of ejaculation? --- Generally, it is not necessary to abstain, but there may still be a small amount of hematospermia.
Wishing you good health.

Post-circumcision concerns, particularly regarding the presence of blood in semen after a period of abstinence, can understandably cause anxiety. Let's address your questions one by one.
1. Is this condition (blood in semen after a long period of abstinence) common?
It is not uncommon for individuals to experience some bleeding in semen (hematospermia) after a period of sexual inactivity, especially following a surgical procedure like circumcision. The body undergoes various changes during the healing process, and the tissues in the genital area can be sensitive. Engaging in sexual activity after a long period of abstinence can sometimes lead to minor trauma or irritation, which may result in blood being present in the semen.
2. Will the blood in semen resolve on its own? If so, how long might it take?
In many cases, hematospermia can resolve on its own, especially if it is related to minor irritation or trauma. The duration for resolution can vary, but if the bleeding is due to irritation from the recent sexual activity, it may take a few days to a week for the symptoms to completely subside. However, if the bleeding persists beyond a week or worsens, it is advisable to seek medical attention.
3. Should you seek medical attention?
Given that you have experienced blood in your semen following a surgical procedure, it is prudent to consult a healthcare provider. While it may be a benign issue, it is essential to rule out any complications related to the circumcision or other underlying conditions. A healthcare professional can perform a thorough evaluation, including a physical examination and possibly imaging or laboratory tests, to ensure that there are no significant issues.
4. Are there any lifestyle considerations? Should you continue abstaining?
If you are not experiencing significant pain or other concerning symptoms, it may be reasonable to continue with moderate sexual activity, but with caution. If you notice any recurrence of bleeding or discomfort, it would be wise to abstain from sexual activity until you have consulted with a healthcare provider. Generally, it is recommended to allow your body some time to heal, especially after a surgical procedure. If you choose to engage in sexual activity, consider doing so gently and gradually to avoid further irritation.
In summary, while the presence of blood in semen after circumcision and a period of abstinence can be concerning, it is often a temporary issue that resolves on its own. However, given the surgical history, it is essential to monitor your symptoms closely and consult a healthcare professional if the bleeding persists or if you have any other concerns. Taking care of your body and allowing adequate time for healing is crucial in the recovery process.
