Menstrual issues
Hello, doctor.
I would like to ask about my last sexual intercourse, which was on April 14, and I used protection.
My normal menstrual cycle usually falls between 28 to 30 days, typically around the 26th to 28th of each month.
My menstrual flow and duration were normal in April and May, but I have not had my period yet as of July 2.
I am feeling a bit anxious and would like to know what possible reasons there might be.
Could it be that my irregular sleep schedule this month (compared to my usual routine) is affecting it?
r, 20~29 year old female. Ask Date: 2024/07/02
Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the factors affecting the menstrual cycle are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraceptives, diet, polycystic ovary syndrome, uterine or cervical lesions, infections, ovulation, pregnancy, and breastfeeding, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Zhang.

Menstrual irregularities can be a source of concern for many women, and understanding the potential causes is crucial for addressing any underlying issues. Based on your description, it appears that you have a regular menstrual cycle, typically occurring every 28-30 days, but you are experiencing a delay in your period for June, which has caused you some anxiety.
There are several potential reasons for menstrual irregularities, including stress, lifestyle changes, hormonal imbalances, and underlying medical conditions. Since you mentioned that your last sexual encounter was on April 14 with protection, it is important to note that while contraceptive methods significantly reduce the risk of pregnancy, they are not 100% effective. If you have missed your period and are concerned about pregnancy, it may be wise to take a home pregnancy test for reassurance.
In addition to pregnancy, several other factors could contribute to your delayed period:
1. Stress: Emotional or physical stress can significantly impact your menstrual cycle. Changes in your routine, such as staying up late or experiencing anxiety, can disrupt the hormonal balance that regulates menstruation.
2. Lifestyle Changes: You mentioned that you have been going to bed later than usual. Sleep patterns play a crucial role in hormonal regulation. Disruptions in sleep can lead to changes in the menstrual cycle.
3. Weight Fluctuations: Significant weight gain or loss can affect your menstrual cycle. If you have experienced any changes in your weight recently, this could be a contributing factor.
4. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can lead to irregular periods. If you have a history of hormonal issues or other symptoms, it may be worth discussing with your healthcare provider.
5. Medical Conditions: Certain medical conditions, including diabetes or other endocrine disorders, can also affect menstrual regularity. If you have any other symptoms, such as excessive fatigue, weight changes, or changes in hair growth, it would be prudent to consult a healthcare professional.
6. Age and Life Stage: If you are in your late teens to early twenties or approaching perimenopause (typically in your late 30s to early 50s), your body may naturally experience fluctuations in your menstrual cycle.
Given that your previous cycles were regular and you are now experiencing a delay, it is advisable to monitor your symptoms closely. If your period does not arrive within a week or if you experience any unusual symptoms, such as severe pain, heavy bleeding, or other concerning signs, it would be wise to consult with a healthcare provider. They can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly imaging studies if necessary.
In summary, while occasional menstrual irregularities can be normal, especially in response to lifestyle changes or stress, persistent irregularities warrant further investigation. Taking a pregnancy test can provide peace of mind, and if the delay continues, seeking medical advice is the best course of action to ensure your reproductive health is in check.
