Panic Disorder Treatment: Side Effects and Medication Efficacy - Psychiatry

Panic disorder treatment issues?

Hello, doctor.
I have experienced a relapse of panic disorder for the third time recently.
I am currently in the fourth week of treatment with Tianeptine Sodium (Taiwanese brand name: 替你憂-S).
At the beginning, taking half a tablet caused side effects such as increased anxiety, heightened panic, and fever.
After about three weeks, the side effects have decreased, but I still occasionally experience sudden feelings of anxiety.
In the fourth week, I adjusted the dosage to one tablet, and recently I have been feeling nauseous and have a poor appetite, but I no longer have a fever.
However, I still experience bouts of anxiety every few days, and panic attacks can still occur suddenly.
Is this because my body is still adjusting to the medication, or does it indicate that the medication is not effective? In my previous two relapses, when I took Lian-Yu, the side effects were less severe, and my panic and anxiety were quickly controlled.
The doctor mentioned that the hospital has changed the medication brand and currently does not have Lian-Yu available, so I was switched to Tianeptine Sodium.
Do different brands of medication really have different side effects or efficacy? Thank you for your response, doctor.

Hello Emily: After taking your medication this time, you are still experiencing occasional panic attacks, which could be due to two possible reasons.
First, your panic disorder may have worsened, making the current dosage of your medication insufficient for complete control.
Second, the efficacy of Tianeptine-S may not be as effective as that of Tianeptine, meaning the same dosage may not achieve the same results as Tianeptine.
Both Tianeptine and Tianeptine-S are generic medications, not brand-name drugs.
Although the active ingredients are the same, there may be differences in other components among different brands, which can affect the medication's efficacy or side effects.
It is advisable to discuss this with your physician to clarify what might be causing your current situation.
Of course, it is also possible that both reasons are contributing.
Wishing you safety and health!

More Info

Panic disorder is a debilitating condition characterized by recurrent and unexpected panic attacks, which can lead to significant distress and impairment in daily functioning. The treatment of panic disorder often involves pharmacotherapy, psychotherapy, or a combination of both. In your case, you are currently taking "替你憂-S" (which is likely a brand name for a medication such as escitalopram or another selective serotonin reuptake inhibitor, SSRI) to manage your symptoms.

Understanding the Medication
1. Efficacy: SSRIs like escitalopram are commonly prescribed for panic disorder due to their effectiveness in reducing anxiety and panic symptoms. They work by increasing the levels of serotonin in the brain, which can help improve mood and reduce anxiety. It is important to note that while SSRIs can be effective, they often take several weeks to reach their full therapeutic effect. This means that the initial weeks of treatment may involve some side effects as your body adjusts to the medication.

2. Side Effects: Common side effects of SSRIs can include nausea, changes in appetite, fatigue, insomnia, and increased anxiety or panic during the initial phase of treatment. It is not uncommon for patients to experience heightened anxiety or panic attacks when starting a new medication or adjusting the dosage. In your case, experiencing increased anxiety and panic during the first few weeks of treatment is a known phenomenon. However, as you have noted, these side effects may diminish over time.

3. Dose Adjustment: You mentioned that your doctor adjusted your dosage to one full tablet after three weeks. This is a common practice as healthcare providers often start patients on a lower dose to minimize side effects and then gradually increase it to find the optimal therapeutic dose. The fact that you are experiencing nausea and decreased appetite at the higher dose could be side effects related to the medication, but they may also be part of your anxiety symptoms.

Medication Variability
You raised an important question regarding the variability of medications from different manufacturers. While the active ingredient in a medication remains the same, the inactive ingredients (excipients) can differ between brands. These differences can sometimes lead to variations in how the medication is absorbed, its efficacy, and the side effects experienced by patients. Therefore, it is possible that switching from "離憂" to "替你憂-S" could result in different side effects or therapeutic responses.

Recommendations
1. Continue Monitoring: It is essential to continue monitoring your symptoms and side effects. If the nausea and appetite changes persist, or if you continue to experience panic attacks, it is crucial to communicate these concerns with your healthcare provider. They may consider adjusting your dosage again or exploring alternative medications.

2. Therapeutic Support: In addition to medication, consider engaging in cognitive-behavioral therapy (CBT) or other forms of psychotherapy. These therapies can provide you with coping strategies to manage panic attacks and anxiety symptoms effectively.

3. Patience with Treatment: Remember that finding the right medication and dosage can take time. It is not uncommon for patients to try several different medications or combinations before finding what works best for them.
4. Emergency Plan: If you experience severe panic attacks or feel overwhelmed, have a plan in place for how to cope during those moments. This might include grounding techniques, breathing exercises, or contacting a trusted friend or therapist.

In conclusion, while it can be frustrating to deal with side effects and fluctuating symptoms, it is important to remain in close contact with your healthcare provider to ensure that you receive the best possible care for your panic disorder. Your experience is valid, and with the right support and treatment adjustments, improvement is achievable.
