Positive pregnancy test after medical abortion?

Hello, doctor.
I had a medical abortion around May 29.
My menstrual period came on June 29, but two days after it ended, on July 8, I experienced bleeding again.
I took a pregnancy test and it showed two lines (the second line is faint).
I had sexual intercourse around June 17 (with contraception).
What could this situation indicate? Is there a significant possibility of pregnancy?

Hua, 20~29 year old female. Ask Date: 2024/07/10

When it comes to the situation of a positive pregnancy test following a medication abortion, there are several important factors to consider. First, it’s essential to understand the timeline of events and the nature of the medications used during the abortion process. Medication abortions typically involve the use of two drugs: mifepristone and misoprostol. These medications work together to terminate a pregnancy, and they are generally effective in doing so. However, in some cases, a follow-up pregnancy test may still show positive results due to residual hCG (human chorionic gonadotropin) levels in the body, which can take time to decrease after an abortion.

In your case, you mentioned that you experienced a menstrual period on June 29, which is a positive sign that your body is returning to its normal cycle following the abortion. However, the subsequent bleeding you experienced on July 8, along with a positive pregnancy test, raises some questions. The faint line on the pregnancy test could indicate a few possibilities:
1. Incomplete Abortion: If the abortion was not fully successful, there may still be some pregnancy tissue remaining in the uterus, which could lead to continued hCG production and a positive pregnancy test. This is something that should be evaluated by a healthcare provider.

2. New Pregnancy: If you had unprotected sexual intercourse around June 17, it is possible that you could have conceived again before your next period. Sperm can survive in the female reproductive tract for several days, so if ovulation occurred shortly after your period ended, conception could have taken place.

3. Hormonal Fluctuations: After a medication abortion, it can take some time for hormone levels to stabilize. This means that even if you are not pregnant, you might still have detectable levels of hCG in your system for a while.

Given these possibilities, it is crucial to consult with a healthcare provider for further evaluation. They may recommend a follow-up ultrasound or blood tests to determine the levels of hCG in your system and assess whether there is any remaining tissue from the abortion or if a new pregnancy has occurred.

In terms of your health and well-being, it’s important to monitor any symptoms you may be experiencing, such as unusual bleeding, cramping, or any signs of infection. If you experience heavy bleeding or severe pain, you should seek medical attention immediately.

In summary, while a positive pregnancy test after a medication abortion can be concerning, it is not uncommon for hCG levels to remain elevated for a period of time. However, given your recent history and the symptoms you are experiencing, it is advisable to consult with your healthcare provider for a thorough evaluation and appropriate management. They can provide you with the necessary guidance and support during this time.
