Intermenstrual bleeding

Hello Dr.
Tsao, I have been experiencing heavy watery bleeding for a week before and after my menstrual period for the past three months.
I need to change my pad at least three to four times a day.
There are no other discharge symptoms, just blood mixed with clear fluid.
Sometimes it is bright red, and occasionally it appears brown with a small amount of blood clots.
Besides the bleeding, I do not feel any pain in other areas, and I have not engaged in sexual intercourse.
I would like to ask the doctor what this situation might be.

Miss Wang, 10~19 year old female. Ask Date: 2024/07/16

Abnormal bleeding in women can be a source of significant concern and confusion, especially when it deviates from the typical menstrual cycle. Based on your description, it appears that you are experiencing heavy bleeding outside of your regular menstrual periods, which can be classified as abnormal uterine bleeding (AUB). Understanding the potential causes and implications of this condition is crucial for addressing your concerns.

Firstly, it is important to recognize that AUB can stem from a variety of factors, including hormonal imbalances, structural abnormalities, and underlying medical conditions. Hormonal fluctuations, particularly involving estrogen and progesterone, can lead to irregularities in the menstrual cycle. For instance, conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can disrupt the normal hormonal balance, resulting in unexpected bleeding.

In your case, the presence of blood mixed with watery discharge, along with the absence of significant pain, suggests that this may not be an acute issue such as an injury or infection. However, it is essential to rule out any serious conditions, such as uterine fibroids, polyps, or even more severe issues like endometrial hyperplasia or malignancies, especially if the bleeding is heavy and persistent.

The color of the blood can also provide insights into the nature of the bleeding. Fresh red blood typically indicates active bleeding, while brown blood often suggests older blood that has taken longer to exit the body. The presence of blood clots can occur in heavier menstrual bleeding, but if you are experiencing larger clots or a significant increase in bleeding, it warrants further investigation.

Given that you have not engaged in sexual activity, the likelihood of pregnancy-related complications is low. However, it is still important to consider other factors that could contribute to your symptoms. Stress, lifestyle changes, and significant weight fluctuations can all impact menstrual regularity and may lead to abnormal bleeding patterns. Additionally, certain medications, including hormonal contraceptives or anticoagulants, can also affect bleeding.

To address your concerns effectively, it is advisable to consult a healthcare provider who specializes in gynecology. They may recommend a thorough evaluation, including a pelvic examination, ultrasound imaging, and possibly blood tests to assess hormone levels and rule out any underlying conditions. A Pap smear may also be indicated to check for cervical health, especially if you have not had one recently.

In terms of management, treatment options will depend on the underlying cause of the abnormal bleeding. If hormonal imbalances are identified, hormonal therapies such as birth control pills may be prescribed to help regulate your cycle and reduce bleeding. In cases where structural abnormalities are found, surgical interventions may be necessary.

In conclusion, while experiencing abnormal bleeding can be distressing, it is essential to approach the situation with a clear understanding of potential causes and the importance of seeking medical advice. By working closely with a healthcare provider, you can obtain a proper diagnosis and develop an appropriate treatment plan tailored to your specific needs. Remember, your health and well-being are paramount, and addressing these symptoms early can lead to better outcomes.
