Issues Related to Sexual Activity or Childbirth After Anal Fistula Surgery

Hello Doctor, I underwent anal fistula surgery in early March, but from May to July, I experienced swelling and even pus formation.
I consulted my original surgeon, who addressed the issues, and after a proctoscopy, it was noted that the internal area had scabbed over.
It was suggested that the recurrence of swelling and pus might be due to an allergy to the absorbable sutures.
I would like to ask:
1.
If the internal wound has not fully healed and is still in the process of healing, should I refrain from any sexual activity with my husband? How long after the surgery should I wait before engaging in sexual activity? (No anal intercourse involved)
2.
During sexual activity, even without anal intercourse, could the act of vaginal penetration affect the healing of the fistula site or cause tearing?
3.
Considering future plans for childbirth, I have experienced some incontinence after the anal fistula surgery.
Would this make vaginal delivery unsuitable for me?
Thank you.

1.
If there is still swelling and purulent discharge, it may indicate a recurrence.
As long as there is no anal intercourse, it should not affect sexual activity.

2.
If the fistula is not connected to the vagina, then it should not be related.

3.
It depends on the subsequent healing of the fistula.
If healing is good and there are no further recurrences, vaginal delivery is not contraindicated.
You can discuss this with your obstetrician.

After undergoing anal fistula repair surgery, it is essential to consider several factors regarding sexual activity and childbirth. Here’s a comprehensive overview addressing your concerns:
1. Timing for Resuming Sexual Activity: Generally, after anal fistula surgery, it is advisable to wait until you have fully healed before resuming any form of sexual activity. This healing period can vary from person to person, but typically, it may take anywhere from 4 to 8 weeks. Engaging in sexual activity too soon can risk reopening the surgical site, leading to complications such as infection or delayed healing. If you are experiencing swelling or discharge, it is a sign that the area may still be healing, and it would be prudent to avoid sexual intercourse until you have been cleared by your healthcare provider.

2. Impact of Sexual Activity on Healing: Even if you are not engaging in anal intercourse, any form of penetration can potentially affect the healing process of the surgical site. The pressure and movement involved in sexual activity could lead to discomfort or even cause the wound to reopen if it is not adequately healed. It is crucial to communicate with your partner about your concerns and to prioritize your healing process. If you have any doubts about the state of your recovery, it is best to consult your surgeon or healthcare provider.

3. Childbirth Considerations: If you are planning to have children in the future, it is essential to discuss your delivery options with your obstetrician. While many women with a history of anal fistula repair can have a vaginal delivery, there are factors to consider, especially if you are experiencing any degree of fecal incontinence. In some cases, a history of anal surgery may increase the risk of complications during vaginal delivery, such as tearing or further injury to the anal sphincter. Your healthcare provider can assess your specific situation and help you determine the safest delivery method for you and your baby.

4. Postoperative Care and Monitoring: After surgery, it is crucial to monitor your recovery closely. If you notice any signs of infection, such as increased pain, swelling, or discharge, or if you experience any new symptoms, you should contact your healthcare provider immediately. Regular follow-up appointments are essential to ensure that your healing is progressing as expected.

5. Psychological and Emotional Considerations: It is also important to address the emotional and psychological aspects of resuming sexual activity after surgery. Many individuals may experience anxiety or fear regarding pain or complications. Open communication with your partner and possibly seeking counseling can be beneficial in navigating these feelings.

In summary, the key to a safe and healthy recovery after anal fistula repair surgery involves patience, proper monitoring of your healing process, and clear communication with your healthcare provider and partner. Always prioritize your health and well-being, and do not hesitate to seek medical advice if you have any concerns about your recovery or future sexual activity and childbirth plans.
