Question

I am 23 years old, and I don't understand why I experience frequent urination and urinary incontinence every time my menstrual period comes.
Normally, I urinate without any issues, but this only happens during my menstrual cycle.
I often run to the bathroom, but the amount of urine is very small each time.
Sometimes, I have leakage that ends up on my sanitary pad, which means I have to change it frequently.
This makes my menstrual period very uncomfortable and affects my daily life.
What could be the reason for this, and what should I do?

JJ, 20~29 year old female. Ask Date: 2004/03/06

Hello: You may need to go to the hospital for further examination to determine if there is endometriosis affecting the bladder or if there are uterine fibroids compressing the bladder.

More Info

Frequent urination and urinary leakage during menstruation can be quite distressing and can significantly impact your quality of life. Understanding the underlying causes and potential solutions is essential for managing these symptoms effectively.

Possible Causes
1. Hormonal Changes: During menstruation, hormonal fluctuations, particularly in estrogen and progesterone, can affect the bladder and urinary tract. These hormones can influence the sensitivity of the bladder, leading to increased urgency and frequency of urination.

2. Pelvic Floor Muscle Changes: The menstrual cycle can also affect the pelvic floor muscles. Some women may experience relaxation or tension in these muscles, which can lead to urinary leakage, especially if there is increased abdominal pressure from cramps or bloating.

3. Increased Fluid Intake: Many women tend to drink more fluids during their menstrual period, whether due to cravings or to alleviate cramps. Increased fluid intake can naturally lead to more frequent urination.

4. Bladder Sensitivity: Some women may have a more sensitive bladder during their menstrual cycle, which can cause them to feel the urge to urinate more frequently, even if their bladder is not full.

5. Urinary Tract Infections (UTIs): Although you mentioned that you usually have no issues with urination outside of your menstrual cycle, it's worth noting that some women may experience UTIs that can flare up during menstruation, leading to increased urgency and frequency.

Recommendations
1. Track Symptoms: Keeping a diary of your symptoms, including the timing of your menstrual cycle, fluid intake, and urination patterns, can help identify any patterns or triggers. This information can be valuable when discussing your symptoms with a healthcare provider.

2. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegel exercises can help improve bladder control and reduce leakage. These exercises involve contracting and relaxing the muscles that control urination.

3. Consult a Healthcare Provider: If your symptoms persist or worsen, it is essential to consult a healthcare provider. They can conduct a thorough evaluation, including a physical examination and possibly tests to rule out conditions such as UTIs or other urinary tract issues.

4. Consider Medication: In some cases, medications that help manage bladder sensitivity or hormonal fluctuations may be appropriate. Discussing this option with your healthcare provider can help determine if it is suitable for you.

5. Lifestyle Modifications: Reducing caffeine and alcohol intake, which can irritate the bladder, may also help manage symptoms. Additionally, practicing relaxation techniques can help alleviate stress, which may contribute to muscle tension and urinary issues.

6. Menstrual Products: Using menstrual products designed for heavy flow or leakage, such as period underwear or pads with higher absorbency, can help you feel more secure and comfortable during your menstrual cycle.

Conclusion
Frequent urination and leakage during menstruation can be attributed to a combination of hormonal changes, pelvic floor muscle dynamics, and possibly increased fluid intake. While these symptoms can be frustrating, there are various strategies and treatments available to help manage them. It is crucial to communicate openly with your healthcare provider to explore the best options tailored to your specific situation. By taking proactive steps, you can improve your comfort and quality of life during your menstrual cycle.

Dr. Lin Manying reply Obstetrics and Gynecology
If there is no pain, only frequent urination, and the symptoms disappear after menstruation, it is likely due to the pressure of uterine contractions on the bladder during the menstrual period.
