Ovulation bleeding
Hello Dr.
Cheng, I would like to ask if the light red or pale red bleeding that starts around day 11 between my menstrual cycles and lasts for nearly a week should be considered ovulation bleeding (since I underwent various examinations two years ago and found no issues with my organs).
Now that I am newly married, I want to know if having intercourse during this bleeding will affect my chances of conception, for example, could the sperm be washed away by the blood? Also, I experience diarrhea before my period, and I also have severe diarrhea around the ovulation period.
Additionally, I feel a sense of heaviness in my lower abdomen; is this considered normal?
Yun Jie, 20~29 year old female. Ask Date: 2004/03/07

Hello: It is normal to experience light bleeding or spotting around the 14th day of the menstrual cycle during ovulation.
The chances of conception are higher during this time, as most sperm will swim toward the uterine cavity, with only a small portion flowing out of the vagina.

More Info
Ovulation bleeding, often referred to as mid-cycle bleeding, is a phenomenon that some women experience around the time of ovulation, typically occurring about 10 to 14 days before the next menstrual period. This bleeding can manifest as light spotting, which may be pink or brown in color, and can last from a few hours to several days. It is generally considered a normal occurrence and is thought to be caused by hormonal fluctuations that occur during the ovulation process.
In your case, experiencing light bleeding around day 11 of your cycle could indeed be classified as ovulation bleeding, especially since you have previously undergone tests that ruled out any significant underlying issues. This type of bleeding is usually not a cause for concern and does not typically affect fertility. The presence of light spotting should not interfere with the chances of conception. The sperm can still reach the egg, and the presence of a small amount of blood is unlikely to wash away sperm or significantly impact their ability to fertilize an egg.
Regarding your concerns about having intercourse during this time, it is important to note that ovulation is the optimal time for conception. If you are ovulating, the chances of becoming pregnant are higher during this period, regardless of the presence of light bleeding. The sperm can survive in the female reproductive tract for several days, so even if there is some bleeding, it is unlikely to hinder the sperm's journey to the egg.
As for the gastrointestinal symptoms you mentioned, such as diarrhea around the time of your menstrual cycle and ovulation, this can also be a common experience for some women. Hormonal changes, particularly fluctuations in estrogen and progesterone, can affect the gastrointestinal system, leading to symptoms like bloating, cramping, and changes in bowel habits. The sensation of a "pulling" or "downward" feeling in the abdomen can also be attributed to ovulation, as the ovaries release an egg and the surrounding tissues may stretch or contract.
If these symptoms are severe or significantly impact your daily life, it may be worth discussing with a healthcare provider. They can help determine if there are any underlying conditions, such as endometriosis or other hormonal imbalances, that might be contributing to your symptoms.
In summary, ovulation bleeding is typically a normal occurrence and should not affect your chances of conception. Engaging in sexual intercourse during this time can still lead to pregnancy, and the presence of light bleeding is not likely to wash away sperm. If you have ongoing concerns about your menstrual cycle, fertility, or gastrointestinal symptoms, it is advisable to consult with a healthcare professional for personalized advice and potential treatment options.
