The issue of ulcers?

Dear Doctor: I previously suffered from hemorrhoids and later recovered by applying medication on my own.
However, I now have sores on the skin around my anus, and sometimes the skin breaks.
What should I do to treat this? What symptoms am I experiencing? Please help me with your answers.
Thank you.

Dr. Jiang Zheen reply Dermatology

In medical terminology, there is no condition known as "rotten window." What you may be experiencing is a boil, which is a bacterial infection that requires treatment with antibiotics.

When dealing with skin ulcers, especially after recovering from hemorrhoids, it is essential to understand the underlying causes and appropriate treatment options. Skin ulcers can arise due to various factors, including pressure, friction, moisture, and even infection. In your case, it seems that the skin in the area may have been compromised during the healing process from hemorrhoids, leading to the development of ulcers.

Firstly, it is crucial to maintain proper hygiene in the affected area. Gently cleanse the skin with mild soap and water, and pat it dry with a clean towel. Avoid scrubbing the area, as this can further irritate the skin and exacerbate the condition. After cleansing, you may want to apply a barrier cream or ointment to protect the skin from moisture and friction. Products containing zinc oxide or petroleum jelly can be beneficial in creating a protective layer.

For the treatment of existing ulcers, topical treatments are often recommended. You can use over-the-counter antibiotic ointments, such as Neosporin, to help prevent infection. If the ulcer appears to be infected (redness, warmth, increased pain, or pus), it is essential to consult a healthcare professional for a more potent antibiotic treatment.

In addition to topical treatments, consider the following strategies to promote healing:
1. Avoid Irritation: Wear loose-fitting clothing and avoid tight underwear that may rub against the affected area. This will help reduce friction and allow the skin to heal.

2. Keep the Area Dry: Moisture can worsen skin ulcers, so it is essential to keep the area dry. If you are prone to sweating, consider using absorbent pads or breathable fabrics.

3. Diet and Hydration: A balanced diet rich in vitamins and minerals can aid in skin healing. Ensure you are well-hydrated, as this supports overall skin health.

4. Avoid Sitting for Prolonged Periods: If possible, try to change positions frequently to relieve pressure on the affected area. Using a cushion or pillow can help distribute weight more evenly.

5. Consider Medical Evaluation: If the ulcers do not improve with home treatment or if they worsen, it is crucial to seek medical attention. A healthcare provider can assess the ulcers and may recommend additional treatments, such as prescription medications or specialized wound care.

6. Monitor for Signs of Infection: Keep an eye on the ulcers for any signs of infection, such as increased redness, swelling, warmth, or discharge. If you notice any of these symptoms, contact your healthcare provider promptly.

7. Follow-Up Care: Regular follow-up appointments with your healthcare provider can help monitor the healing process and adjust treatment as necessary.

In summary, skin ulcers following hemorrhoid recovery can be managed effectively with proper hygiene, protective ointments, and lifestyle adjustments. If the condition persists or worsens, do not hesitate to seek professional medical advice. Your healthcare provider can offer tailored recommendations based on your specific situation, ensuring a more effective healing process.
