Irregular menstruation

Hello, I have been experiencing irregular menstruation since I started my period in the seventh grade, and now at 23 years old, my cycles have never been normal.
They come approximately every two to three months; sometimes I have a period in one month, sometimes in two months, and the longest I have gone is over three months without a period.
Each time I do have my period, it is very painful.
I have consulted many traditional Chinese and Western medicine practitioners, but nothing seems to help.
I am gradually becoming indifferent to it.
What kind of treatment should I pursue to normalize my cycles? Additionally, will this affect my chances of conception compared to others? Thank you.

Jiang Xiaojie, 20~29 year old female. Ask Date: 2004/03/14

Dr. Zhou Dali reply Obstetrics and Gynecology

Hello Ms.
Jiang: The menstrual cycle can vary depending on individual physiology.
The chances of conception need to be discussed in conjunction with other examinations.

Irregular menstruation, also known as oligomenorrhea, can be a complex issue with various underlying causes. It is essential to understand that menstrual cycles can be influenced by a multitude of factors, including hormonal imbalances, lifestyle choices, stress levels, and underlying medical conditions. Given your history of irregular periods since adolescence, it is crucial to explore these potential causes further.

Causes of Irregular Menstruation
1. Hormonal Imbalances: The menstrual cycle is regulated by a delicate balance of hormones, including estrogen and progesterone. Conditions such as polycystic ovary syndrome (PCOS) can lead to hormonal imbalances, resulting in irregular periods. PCOS is characterized by enlarged ovaries containing small cysts and can also cause symptoms like weight gain, acne, and excessive hair growth.

2. Stress and Lifestyle Factors: High levels of stress can significantly impact your menstrual cycle. Stress triggers the release of cortisol, which can interfere with the hormones responsible for regulating your cycle. Additionally, lifestyle factors such as diet, exercise, and sleep patterns can also play a role. For instance, excessive exercise or significant weight fluctuations can disrupt your menstrual cycle.

3. Medical Conditions: Certain medical conditions, such as thyroid disorders (hypothyroidism or hyperthyroidism) and uterine abnormalities (like fibroids or polyps), can also lead to irregular menstruation. It is essential to rule out these conditions through appropriate medical evaluations.

4. Age and Development: Since you mentioned that your irregular periods started in middle school, it is possible that your body is still adjusting to hormonal changes during puberty. However, at 23, it is crucial to seek a more definitive diagnosis and treatment plan.

Solutions for Better Health
1. Consult a Specialist: Since you have already seen various healthcare providers without significant improvement, consider consulting a reproductive endocrinologist or a gynecologist specializing in menstrual disorders. They can conduct specific tests, such as hormone level assessments, ultrasounds, or other diagnostic procedures, to identify the underlying cause of your irregular periods.

2. Lifestyle Modifications: Implementing healthy lifestyle changes can have a positive impact on your menstrual health. Focus on maintaining a balanced diet rich in whole foods, fruits, vegetables, and lean proteins. Regular physical activity can help regulate hormones and reduce stress levels. Additionally, prioritize sleep hygiene and stress management techniques, such as yoga or meditation.

3. Medications and Treatments: Depending on the underlying cause of your irregular periods, your healthcare provider may recommend hormonal treatments, such as birth control pills, to help regulate your cycle. If PCOS is diagnosed, lifestyle changes combined with medications like metformin may be beneficial.

4. Monitoring and Tracking: Keeping a menstrual diary can help you and your healthcare provider identify patterns in your cycle and any associated symptoms. This information can be invaluable in tailoring a treatment plan that works for you.

Impact on Fertility
Regarding your concern about fertility, irregular menstruation can indeed affect your chances of conception. Irregular cycles may indicate that ovulation is not occurring regularly, which can make it more challenging to conceive. However, many women with irregular periods can still become pregnant, especially with appropriate medical intervention and lifestyle changes.

In conclusion, while irregular menstruation can be frustrating and concerning, there are various avenues to explore for diagnosis and treatment. By working closely with a healthcare provider and making informed lifestyle choices, you can improve your menstrual health and overall well-being. Don't hesitate to advocate for yourself and seek the care you need to achieve a regular cycle and enhance your fertility prospects.
