Penile issues..?

Hello, doctor! I have a question.
Recently, I was playing basketball with friends and accidentally got hit in the penis.
At that moment, I felt numbness, but it got better after resting at home.
However, urination doesn't seem as smooth as before; it takes about 7 to 8 seconds before I can start.
Will there be any long-term effects? Could this cause any damage to my kidneys?

kc, 20~29 year old female. Ask Date: 2004/03/22

Hello KC, regarding the accidental injury to your penis during exercise, I would like to know if you experienced any swelling or bruising at the time of the injury.
If so, it indicates a more significant injury and a higher likelihood of traumatic urethral stricture.
However, if there were no obvious external signs of injury, the possibility of traumatic urethral stricture is unlikely.
Severe urethral stricture, if left untreated for a long time, can lead to excessive retrograde pressure, potentially causing hydronephrosis and kidney damage.
Given your current situation, I recommend undergoing a urethrocystoscopy to obtain a clear diagnosis regarding the presence of urethral stricture and whether treatment is necessary.

Penile injuries, while not commonly discussed, can occur during sports or physical activities, and they can lead to various complications depending on the severity of the trauma. In your case, it sounds like you experienced a direct impact to the penis, which can cause bruising, swelling, or even more serious injuries to the surrounding tissues.
The initial sensation of numbness you felt after the injury is not unusual. It may have been due to temporary nerve compression or trauma to the area. However, the fact that you are experiencing a delay in urination (taking 7-8 seconds to start) is concerning and warrants further evaluation. This could indicate that there is some degree of injury to the urethra, which is the tube that carries urine from the bladder out of the body.
Injuries to the penis can lead to complications such as:
1. Urethral Injury: If the urethra is damaged, it can lead to difficulties in urination, blood in the urine, or even urinary retention. This is a serious condition that requires immediate medical attention.

2. Penile Fracture: Although this term is often used to describe a rupture of the tunica albuginea (the fibrous covering of the erectile tissue), it can occur with significant trauma. Symptoms include sudden pain, swelling, and sometimes a noticeable deformity.

3. Nerve Damage: Trauma can also affect the nerves in the area, leading to numbness or altered sensation. This may resolve over time, but in some cases, it can lead to persistent issues.

4. Psychological Impact: Injuries to the genital area can also have psychological effects, including anxiety about sexual function or performance.

Regarding your concern about kidney damage, it is unlikely that a direct impact to the penis would cause harm to the kidneys unless there was significant trauma to the lower abdomen or pelvis. However, if you experience any blood in your urine, severe pain, or changes in urinary habits, it is essential to seek medical attention promptly.

In terms of recovery, most minor penile injuries heal well with conservative management, which includes:
- Rest: Avoid activities that could exacerbate the injury.

- Ice: Applying ice packs can help reduce swelling and pain.

- Pain Relief: Over-the-counter pain medications like ibuprofen or acetaminophen can be helpful.

- Hydration: Ensure you are well-hydrated, which can help with urination.

If your symptoms persist or worsen, it is crucial to consult a healthcare provider. They may recommend imaging studies or a urological evaluation to rule out any significant injuries.
In summary, while many penile injuries can heal without long-term consequences, the symptoms you describe suggest that a thorough evaluation is warranted to ensure there are no serious underlying issues. Don't hesitate to reach out to a healthcare professional for a proper assessment and peace of mind.
