What disease is this?
Hello Dr.
Tseng, I currently have symptoms that suggest a cold, but after being diagnosed and having blood tests done by an otolaryngologist, it appears to be an issue with my immune system.
I have swelling in the area under my jaw and neck that is painful when pressed, and I have experienced mild fevers for several days.
There are several nodular protrusions in my throat, and despite taking antibiotics, there has been no reduction in swelling.
I have a history of allergies and have been dealing with urticaria for some time.
Could you please advise me on what my condition might be and how I should proceed with treatment? Thank you!
Miss Lin, 20~29 year old female. Ask Date: 2004/03/25
Dr. Zeng Guosen reply Internal Medicine
Hello: Swelling and tenderness in the area below the ear and neck, along with fever, are most likely indicative of an infection combined with lymphadenopathy.
This is less likely related to an allergic condition.
It is recommended to visit an infectious disease outpatient clinic.
Sincerely, Dr.
Tseng Kuo-Sen, Department of Internal Medicine, Taoyuan Hospital, Ministry of Health and Welfare.

More Info
It sounds like you are experiencing a complex set of symptoms that may indeed be related to an underlying issue with your immune system. Given your history of allergies and the recent diagnosis from your ENT specialist, it is important to consider several factors that could be contributing to your current condition.
Firstly, the presence of swollen lymph nodes in the neck, especially if they are painful, can indicate an immune response to an infection or inflammation. In your case, the fact that you have been experiencing mild fevers for several days suggests that your body is fighting something off. This could be a viral or bacterial infection, or it could be related to your immune system's response to allergens, especially considering your history of allergies and urticaria (hives).
The growth of nodular masses in your throat could be indicative of several conditions, including viral infections like infectious mononucleosis, bacterial infections such as strep throat, or even more chronic conditions like lymphadenopathy due to autoimmune disorders. Since you mentioned that you have been on antibiotics without improvement, it is possible that the cause of your symptoms is not bacterial in nature, which would explain the lack of response to antibiotic treatment.
Your allergic predisposition may also play a role in your symptoms. Allergies can lead to chronic inflammation, which can cause swelling of lymph nodes and other tissues. The fact that you have been dealing with urticaria suggests that your immune system may be overreacting to certain stimuli, which could also be contributing to your current symptoms.
In terms of treatment, it is crucial to follow up with your healthcare provider for further evaluation. They may recommend additional tests, such as imaging studies (like an ultrasound of the neck) or blood tests to assess for markers of inflammation or autoimmune activity. If your symptoms persist or worsen, a referral to a specialist, such as a rheumatologist or an allergist, may be warranted to explore potential underlying autoimmune conditions.
In the meantime, managing your symptoms is key. Over-the-counter antihistamines may help alleviate some of the allergic symptoms, and staying hydrated is important, especially if you have a fever. If you experience significant pain or discomfort, your doctor may prescribe pain relievers or corticosteroids to reduce inflammation.
It is also essential to monitor your symptoms closely. If you experience difficulty breathing, severe swelling, or any other alarming symptoms, seek medical attention immediately. Your health is paramount, and addressing these symptoms early can help prevent complications.
In summary, while your symptoms may suggest an issue with your immune system, a thorough evaluation by your healthcare provider is necessary to determine the exact cause and appropriate treatment. Keep track of your symptoms, follow your doctor's recommendations, and do not hesitate to seek further medical advice if your condition does not improve.
