Hello, I would like to inquire about the situation after an appendectomy?
Hello, I had surgery on March 3 for acute appendicitis and was discharged on March 11.
The wound healing seems to be going well, but I noticed a hard lump below the incision.
On March 27, I discovered blood in my stool, and there was also blood on the toilet paper.
On March 29, it happened again.
Is this a normal occurrence, or could it indicate that the internal wound has reopened? Please advise me as soon as possible.
Thank you.
Lin, 20~29 year old female. Ask Date: 2004/03/29

Reply to Mr.
Lin: The surgical wound appears normal on the surface, but there is a hard lump underneath, which may be a normal part of the healing process or could indicate an abnormal blood clot or abscess.
A differential diagnosis is necessary.
Regarding the presence of blood in the stool, it is important to determine whether this is related to pre-existing gastrointestinal issues or if it is associated with the recent surgery.
It is advisable to return to the original hospital where the surgery was performed for further evaluation and clarification, in order to facilitate appropriate management.

After an appendectomy, it is not uncommon for patients to experience a variety of symptoms as they recover. However, the presence of a hard lump and blood in the stool, as you described, raises some important concerns that should be addressed promptly.
Firstly, the hard lump you feel beneath the incision site could be a few different things. It might be a hematoma, which is a localized collection of blood outside of blood vessels, often caused by bleeding during or after surgery. Hematomas can feel firm or hard to the touch and may resolve on their own over time. Another possibility is that it could be scar tissue or a seroma, which is a pocket of clear fluid that sometimes forms after surgery. While these are generally not serious, they should be monitored for changes in size or symptoms.
The presence of blood in your stool is more concerning. This could indicate several issues, including:
1. Gastrointestinal Bleeding: Blood in the stool can arise from various sources in the gastrointestinal tract, including the stomach, intestines, or rectum. Given your recent surgery, it is crucial to determine whether the bleeding is related to the surgical site or if it is due to another underlying condition.
2. Infection or Complications: After an appendectomy, there is a risk of infection or complications such as an abscess forming in the abdominal cavity. If the surgical site has become infected, it could potentially lead to bleeding.
3. Bowel Injury: Although rare, there is a possibility of an injury to the bowel during surgery, which could lead to bleeding. This is a serious complication that requires immediate medical attention.
Given these potential issues, it is essential to seek medical attention as soon as possible. You should contact your surgeon or go to the nearest emergency room to have your symptoms evaluated. They may perform a physical examination and possibly imaging studies, such as an ultrasound or CT scan, to assess the lump and check for any internal bleeding or complications.
In the meantime, monitor your symptoms closely. If you experience increased pain, fever, or any signs of shock (such as rapid heartbeat, weakness, or confusion), seek emergency care immediately.
In summary, while some post-operative symptoms can be normal, the combination of a hard lump and blood in your stool is concerning and warrants prompt medical evaluation. It is always better to err on the side of caution when it comes to post-surgical complications. Your health and safety are the top priority, and a healthcare professional will be able to provide the necessary assessments and interventions.
