Abnormal bleeding during menstruation?
Hello, Doctor! I would like to ask you about something.
Over the past year, I have been experiencing irregular bleeding during my menstrual cycle, which is typically 28 days.
The bleeding occurs between days 10 to 15, and there is fresh blood each time, along with some brown discharge.
My previous boyfriend attempted to ejaculate at my vaginal opening, but we never had actual sexual intercourse.
Therefore, when I consulted a doctor, they advised against a Pap smear, stating that it could rupture my hymen.
I have had a pelvic ultrasound, and both my ovaries and uterus appeared normal.
The doctor mentioned that it could be ovulation bleeding, but they couldn't be completely certain since I haven't had a Pap smear.
Additionally, I have had several yeast infections in the past, and I am concerned about the possibility of cervical cell changes and cervical cancer.
Doctor, do you think it is worth it for me to undergo a Pap smear? (Since the doctor mentioned it could lead to the rupture of my hymen.) Are there any other methods besides a Pap smear to check for abnormalities in the cervix? Thank you, Doctor!

Dear user,
Based on your description of the menstrual cycle, abnormal bleeding duration, and the nature of the abnormal bleeding, I also believe it is likely non-pathological, possibly slight bleeding during ovulation.
Of course, common abnormal uterine bleeding should consider the possibility of other uterine pathologies.
Since you have already undergone a gynecological ultrasound examination and both your ovaries and uterus are normal, there is no need for excessive concern.
It has been confirmed that multiple sexual partners, complex sexual relationships, and human papillomavirus (HPV) infections are significant causes of cervical cancer.
Recurrent candidiasis is not related to the risk of cervical cancer.
Women who have never engaged in vaginal intercourse are unlikely to develop vaginitis or pelvic inflammatory disease.
Your previous experience with candidal vaginitis may have been due to non-penetrative vaginal contact, which could have posed a risk of infection.
Using a vaginal speculum for a Pap smear can disrupt the integrity of the hymen; however, if necessary, a fine cotton swab can be carefully inserted through the hymenal opening to collect a sample from the cervix for the Pap test.

More Info
Abnormal bleeding during menstruation can be concerning, especially when it deviates from your typical cycle. In your case, experiencing bleeding between days 10 to 15 of your 28-day cycle, accompanied by fresh blood and brown discharge, raises several possibilities that warrant further exploration.
Firstly, it is important to understand that mid-cycle bleeding can sometimes be attributed to ovulation. Ovulation bleeding occurs when an egg is released from the ovary, which can cause a small amount of bleeding due to the rupture of the follicle. This type of bleeding is usually light and lasts for a short duration. However, since you have been experiencing this consistently over the past year, it is essential to consider other potential causes.
Hormonal imbalances are another common reason for abnormal bleeding. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or even stress can lead to irregularities in your menstrual cycle. Additionally, if you have a history of yeast infections, it’s worth noting that chronic inflammation or irritation in the vaginal area can sometimes lead to abnormal bleeding.
Regarding your concern about cervical cancer, it is understandable to be anxious, especially if you have had previous infections. While your doctor may have advised against a Pap smear due to concerns about your hymen, it is crucial to weigh the risks and benefits of such a test. A Pap smear is a vital screening tool for detecting precancerous changes in cervical cells and can help rule out cervical cancer. If you are concerned about the integrity of your hymen, it is worth discussing this openly with your healthcare provider. They can provide guidance on how to proceed safely.
In addition to a Pap smear, there are other diagnostic methods to assess cervical health. A colposcopy, for instance, allows for a closer examination of the cervix using a special magnifying instrument. This procedure can be performed if there are any abnormalities detected during a Pap smear or if your doctor feels it is necessary based on your symptoms. Furthermore, transvaginal ultrasound can provide additional information about the reproductive organs, although it seems you have already undergone this without any abnormalities noted.
It is also essential to maintain open communication with your healthcare provider. If you feel uncomfortable with their recommendations or have ongoing concerns, seeking a second opinion can be beneficial. Your health and peace of mind are paramount, and you deserve to have your concerns addressed thoroughly.
In summary, while mid-cycle bleeding can be benign, the persistence and nature of your symptoms warrant further investigation. Discussing the possibility of a Pap smear or alternative diagnostic methods with your healthcare provider is crucial. Additionally, consider lifestyle factors that may contribute to hormonal imbalances and ensure you are receiving comprehensive care for any underlying conditions. Remember, early detection and intervention are key in managing reproductive health effectively.
