There is severe itching in the genital area?
Hello, the skin on my lower body, particularly in hairy areas, is very itchy.
The more I scratch, the itchier it gets, and I notice some small black dots appearing under the hair.
Sometimes, a small amount of fluid oozes out.
What should I do? Is this related to herpes? I tend to develop a cluster of small blisters in the same area every few months, which eventually dry up and form scabs after a few days.
Thank you!
Bob, 40~49 year old female. Ask Date: 2004/04/27
Dr. Huang Ruiyun reply Dermatology
You may have eczema, but you still need to see a doctor for a definitive diagnosis.
The issue with the second blister is likely herpes; unfortunately, herpes cannot be completely eradicated and tends to recur after some time.

More Info
It sounds like you're experiencing a troubling skin condition in your genital area, characterized by intense itching, the appearance of small black dots, and occasional fluid discharge. Based on your description, there are several potential causes for these symptoms, and understanding them is crucial for effective treatment.
Firstly, the intense itching and the formation of small blisters or bumps could be indicative of several conditions, including folliculitis, contact dermatitis, or even a fungal infection. Folliculitis is an inflammation of the hair follicles, often caused by bacteria or fungi, which can lead to itchy, red bumps that may sometimes ooze. Contact dermatitis could result from an allergic reaction to soaps, detergents, or other irritants that come into contact with the skin. Fungal infections, such as tinea cruris (commonly known as jock itch), can also cause itching and discomfort in the groin area.
The small black dots you mentioned could be ingrown hairs, especially if you have coarse hair in that area. Ingrown hairs occur when hair grows back into the skin instead of out of it, leading to inflammation and sometimes infection. If you are experiencing fluid discharge, it could suggest a secondary infection or a more serious condition that requires medical attention.
Regarding your question about herpes, while herpes can cause blister-like lesions in the genital area, the pattern you've described—itching followed by the formation of small blisters that eventually crust over—could also be consistent with other conditions. It is essential to differentiate between these possibilities, as the treatment would vary significantly depending on the underlying cause.
For immediate relief, consider the following steps:
1. Avoid Scratching: While it may be tempting, scratching can exacerbate the irritation and lead to further inflammation or infection.
2. Keep the Area Clean and Dry: Gently wash the area with mild soap and water, and pat it dry. Avoid using harsh soaps or products that can irritate the skin.
3. Use Over-the-Counter Treatments: You might find relief from topical treatments such as hydrocortisone cream for inflammation or antifungal creams if a fungal infection is suspected. However, it's crucial to use these products as directed and not for prolonged periods without medical advice.
4. Wear Loose Clothing: Tight clothing can irritate the skin further. Opt for breathable, loose-fitting underwear made from natural fibers.
5. Consult a Healthcare Provider: Given the recurring nature of your symptoms and the potential for various underlying causes, it is advisable to see a healthcare professional. They can perform a physical examination, possibly conduct tests (like a swab of the affected area), and provide a definitive diagnosis and treatment plan.
In terms of prevention, maintaining good hygiene, avoiding known irritants, and managing any underlying skin conditions can help reduce the frequency of flare-ups. If you have a history of similar symptoms, keeping a journal of your outbreaks, including potential triggers, can be beneficial for your healthcare provider.
In conclusion, while your symptoms could relate to several conditions, a thorough evaluation by a healthcare professional is essential for accurate diagnosis and effective treatment. Don't hesitate to seek medical advice, especially if your symptoms persist or worsen.
